Açık Femoral Herni Onarımı
Özet
Femoral herni, peritoneal bir kesenin femoral kanaldan dışarı çıkmasıyla oluşan ve kadınlarda yaşla birlikte sıklığı artan bir kasık fıtığı türüdür. Tanı konulduğu anda yüksek strangülasyon (boğulma) ve inkarserasyon riski nedeniyle acil veya bekletilmeden cerrahi müdahale gerektirir. Tedavisinde klasik doku onarımları olan subinguinal (düşük), inguinal (yüksek) ve preperitoneal (ekstraperitoneal) yaklaşımlar kullanılır. Günümüzde nüks oranlarını düşürmek amacıyla açık mesh (yama) veya plug mesh (tıkaç yama) uygulamaları preperitoneal onarımla birlikte daha çok tercih edilmektedir. Strangülasyon geliştiğinde özellikle ince bağırsak canlılığının kontrol edilmesi kritik önem taşır.
Femoral hernia is a type of groin hernia formed by the protrusion of a peritoneal sac through the femoral canal, with its incidence increasing with age in women. Due to the high risk of strangulation and incarceration, it requires immediate surgical intervention upon diagnosis. Treatment methods involve classic tissue repairs utilizing subinguinal (low), inguinal (high), and preperitoneal (extraperitoneal) approaches. Today, open mesh or plug mesh applications are widely preferred alongside preperitoneal repair to reduce recurrence rates. When strangulation occurs, assessing the viability of the incarcerated loops, particularly the small intestine, is of critical importance.
