Transabdominal Preperitoneal (TAPP) Herni Onarımı
Özet
Minimal invaziv kasık fıtığı onarımlarından biri olan TransAbdominal PrePeritoneal (TAPP) yöntemi, preperitoneal alan ile transvers fasya arasındaki plandan yararlanarak gerçekleştirilen popüler bir tekniktir. TAPP onarımı, intraperitoneal alana girilmesi sayesinde cerraha herhangi bir diseksiyona gerek kalmadan çift taraflı fıtıkları tespit etme ve özellikle boğulmuş fıtıklarda fıtık kesesi içindeki organları kolayca değerlendirme avantajı sunar. Genellikle genel anestezi altında ve profilaktik antibiyoterapi eşliğinde uygulanan bu ameliyatta, hastanın pozisyonu ve trokar yerleşimleri titizlikle planlanır. Cerrahi diseksiyon lateralde iliopubik traktın başlangıcından başlayarak Bogros ve mediyalde Retzius boşluklarına ulaşılmasıyla devam eder. Direkt, indirekt veya femoral fıtık keseleri diseke edilirken spermatik kord yapıları ayrıştırılır. Bu aşamada, ciddi peroperatif kanama riskine yol açabilen "Triangle of Doom" (Kıyamet Üçgeni) ve "Corona Mortis" ile postoperatif kronik ağrı riski taşıyan "Triangle of Pain" (Ağrı Üçgeni) gibi kritik anatomik bölgelerden mesh tespiti sırasında kesinlikle uzak durulmalıdır. Diseksiyonun ardından, tüm defektleri kapatacak boyutta (yaklaşık 10x15 cm) büyük porlu bir mesh düzgünce serilir ve olabildiğince az tespitleyiciyle sabitlenir. Operasyon, mesh migrasyonunu ve intestinal obstrüksiyon gibi komplikasyonları önlemek amacıyla periton fleplerinin intrakorporal devamlı sütürlerle kapatılması ve trokar yerlerinin dikilmesiyle sonlandırılır. TAPP, batın içi geniş görüş açısı ve bilinen laparoskopik planlara benzerliği sayesinde daha kısa bir öğrenme eğrisine sahiptir.
The TransAbdominal PrePeritoneal (TAPP) repair, a minimal invasive inguinal hernia technique, utilizes the dissection plane between the preperitoneal space and the transversalis fascia. Entering the intraperitoneal space allows for the detection of bilateral hernias without additional dissection and enables easy evaluation of intra-abdominal organs, particularly in cases of incarceration. Performed under general anesthesia with prophylactic antibiotic therapy, the procedure requires precise patient positioning and trocar placement. Dissection begins laterally at the iliopubic tract, progressing through Bogros and medial Retzius spaces to expose the Cooper ligament. After dissecting direct, indirect, or femoral hernia sacs from spermatic cord structures, specific hazardous anatomical zones must be avoided during mesh fixation: the "Triangle of Doom" and "Corona Mortis" due to severe bleeding risks, and the "Triangle of Pain" due to chronic postoperative pain. A large-pore mesh (approximately 10x15 cm) is then deployed flatly over all potential defects and secured with minimal tackers. Finally, to prevent mesh migration and intestinal obstruction, the peritoneal flaps are closed intracorporally with continuous sutures before port site closure. Providing an intra-abdominal view, TAPP offers a shorter learning curve for surgeons with sound anatomical knowledge.
