Özet
Laparoskopik total ekstraperitoneal (TEP) kasık fıtığı onarımı, karın boşluğuna girmeden preperitoneal alandan gerçekleştirilen, son derece etkili ve minimal invaziv bir cerrahi yöntemdir. Yaşam boyu görülme sıklığı erkeklerde %27, kadınlarda %3 olan kasık fıtıklarının tedavisinde, açık yöntemlere kıyasla daha az postoperatif ağrı, kozmetik üstünlük ve işe daha hızlı dönüş gibi belirgin avantajlar sunar. Bu prosedür, pubik simfizis ve Cooper ligamentinin tanımlanması, Hesselbach üçgeni ile herni alanlarının belirlenmesi, epigastrik damarların ayrılması, Bogros alanının açılması, kord yapılarının diseksiyonu ve uygun polipropilen ağın (mesh) yerleştirilmesi olmak üzere yedi temel adımdan oluşur. Cerrahi başarı için inguinal anatomiye tam hakimiyet ve öğrenme eğrisinin tamamlanması kritik önem taşır. Operasyon sırasında mesane, bağırsak, kord ve hayatı tehdit edebilecek major vasküler yaralanmaların yanı sıra kronik ağrıya yol açabilecek sinir hasarları gibi ciddi komplikasyon riskleri mevcuttur. Özellikle "ölüm üçgeni" ve "ağrı üçgeni" olarak tanımlanan anatomik bölgelerde mesh tespiti (tacker) yapılmasından kaçınılması, hasta güvenliği ve postoperatif konfor açısından büyük önem arz eder.
Laparoscopic totally extraperitoneal (TEP) inguinal hernia repair is a highly effective, minimal invasive surgical technique performed in the preperitoneal space without entering the abdominal cavity. Providing significant advantages over open surgeries, such as less postoperative pain, superior cosmetic outcomes, and a quicker return to daily activities, it targets a disease with a lifetime prevalence of 27% in men and 3% in women. The procedure consists of seven fundamental steps: identifying the pubic symphysis and Cooper's ligament, defining the Hesselbach's triangle along with potential hernia sites, isolating the epigastric vessels, developing the Bogros space, dissecting the cord structures, and deploying a proper polypropylene mesh. Complete mastery of inguinal anatomy and fulfilling the learning curve are crucial for surgical success. During the operation, serious complication risks exist, including bladder, bowel, cord, and life-threatening major vascular injuries, as well as nerve damages leading to chronic pain. Avoiding mesh fixation with tackers in specific anatomical zones classified as the "triangle of doom" and "triangle of pain" is of paramount importance to maximize patient safety and postoperative comfort.
