Özet

Halk sağlığı çalışmalarında önemli bir yere sahip olan tiroid hastalıkları, özellikle hipotiroidizm tablosuyla bilişsel kapasiteyi olumsuz etkilediği için ayrı bir öneme sahiptir. Dünyada ve Türkiye'de iyot eksikliği ile konjenital hipotiroidi, sık görülmesi ve önlenebilir olması nedeniyle kritik bir sağlık sorunu olmayı sürdürmektedir. İyot eksikliği tedavi edilebilir olduğundan, erken müdahale edilmediğinde endemik guatrdan zekâ geriliğine kadar ciddi tablolara yol açmaktadır. Tarihsel süreçte binlerce yıl öncesine dayanan tiroid tedavileri, 1811'de iyotun keşfi ve ardından hormon izolasyonlarıyla modern döneme taşınmıştır. Dünya genelinde konjenital hipotiroidi prevalansı 1/3000 kabul edilmekle birlikte, Türkiye sıklığın yüksek olduğu bölgeler arasında yer almaktadır. Bu doğrultuda ülkemizde 2006 yılında başlatılan Neonatal Tarama Programı kapsamında, bebeklerin topuğundan alınan kan numuneleriyle erken teşhis ve tedavi protokolleri başarıyla uygulanmaktadır. Küresel düzeyde laboratuvar güvence programları ve tuzların zorunlu iyotlanması gibi önleme faaliyetleri yürütülse de az gelişmiş bölgelerde lojistik, finansman ve eğitim eksiklikleri nedeniyle tarama programlarında belirgin eşitsizlikler yaşanmaktadır.

Thyroid diseases hold a significant place in public health studies, with hypothyroidism conditions being particularly crucial as they directly reduce cognitive capacity. Iodine deficiency and congenital hypothyroidism remain critical global and national health concerns due to their high prevalence and preventable nature. Since iodine deficiency is fully treatable, early intervention can successfully prevent severe outcomes ranging from endemic goiter to mental retardation. The history of thyroid treatment dates back thousands of years, transitioning into modern medicine with the discovery of iodine in 1811 and subsequent thyroid hormone isolations. While the global prevalence of congenital hypothyroidism is estimated at 1/3000, Turkey is identified as a high-frequency region. Consequently, Turkey's Neonatal Screening Program, launched in 2006, utilizes heel prick blood samples to ensure timely diagnosis and treatment protocols. Although global preventative actions like quality assurance programs and mandatory salt iodization are widely implemented, significant disparities in screening coverage persist in underdeveloped regions due to challenges in logistics, funding, and public awareness.
