Tiroid Fonksiyon Bozuklarında Kas İskelet Sistemi Bulguları
Özet
Tiroid fonksiyon bozuklukları ile kas iskelet sistemi arasındaki yakın ilişki, endokrin sistem patolojilerinin romatizmal ve nöromüsküler klinik bulgularla ne denli iç içe geçtiğini göstermektedir. Hipotiroidi vakalarında artropati, proksimal miyopati, karpal tünel sendromu (KTS), Dupuytren kontraktürü ve adeziv kapsülit (donuk omuz) gibi rahatsızlıklar sıklıkla karşımıza çıkmaktadır. Özellikle hyalüronik asit birikimi, eklem efüzyonlarına ve tünel içi hacmi artırarak median sinir sıkışmasına yol açmaktadır. Hipertiroidi durumunda ise osteoblastik ve osteoklastik aktivitelerin hızlanması yıkım lehine sonuçlanarak kemik mineral yoğunluğunda düşüşe, erken osteoporoza ve vertebra kırıklarına neden olmaktadır. Bununla birlikte hipertiroidide kas güçsüzlüğü, periartritler, nadir görülen tiroid akropatisi ve acil bir durum olan tirotoksik hipokalemik periyodik paralizi tablosu gelişebilmektedir. Son olarak romatoid artrit (RA), sistemik lupus eritematozus (SLE), Sjögren sendromu ve fibromiyalji gibi otoimmün romatizmal hastalıklar ile Haşimato ve Graves gibi otoimmün tiroid hastalıkları arasında yüksek bir birliktelik oranı ve ortak immunolojik mekanizmalar mevcuttur. Kollajen doku ve romatizmal hastalıkların takibinde tiroid testlerinin rutin olarak yapılmasının, eşlik eden patolojilerin erken tanısı ve etkin tedavi planlaması açısından kritik önem taşıdığı vurgulanmaktadır.
The close relationship between thyroid dysfunctions and the musculoskeletal system demonstrates how deeply endocrine pathologies intertwine with rheumatic and neuromuscular clinical findings. In cases of hypothyroidism, manifestations such as arthropathy, proximal myopathy, carpal tunnel syndrome (CTS), Dupuytren's contracture, and adhesive capsulitis (frozen shoulder) are frequently encountered. Specifically, the accumulation of hyaluronic acid leads to joint effusions and median nerve compression by increasing pressure within the carpal tunnel. Conversely, hyperthyroidism accelerates both osteoblastic and osteoclastic activities in favor of bone resorption, causing a decrease in bone mineral density, early osteoporosis, and vertebral fractures. Additionally, muscle weakness, periarthritis, rare thyroid acropachy, and thyrotoxic hypokalemic periodic paralysis—a medical emergency—can develop during hyperthyroidism. Lastly, a high prevalence of coexistence and shared immunologic mechanisms exists between autoimmune rheumatic diseases, such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), Sjögren's syndrome, and fibromyalgia, and autoimmune thyroid diseases like Hashimoto's and Graves'. Routine thyroid screening in the follow-up of connective tissue and rheumatic diseases is strongly emphasized as critical for early diagnosis and effective treatment planning of these concurrent pathologies.
