Özet

Tiroid fonksiyon bozuklukları, periferik ve santral sinir sistemini etkileyerek çeşitli nörolojik belirtilere yol açabilen yaygın endokrin hastalıklardır. Hipertiroidi; bilişsel bozukluklar, hafıza kaybı, tremor, nadiren kore ve kas güçsüzlüğü ile karakterize miyopatilere neden olabilir. Yaşamı tehdit eden tiroid fırtınası durumunda ise deliryum, nöbet ve komaya ilerleyen ağır bir nörolojik tablo gelişebilir. Hipotiroidi ise dikkat azalması, düşünce yavaşlaması ve kısa süreli bellek bozuklukları gibi hafif kognitif gerilemelerden demansa kadar uzanan geniş bir yelpazede bilişsel etkiler gösterir. Ayrıca hipotiroidide karpal tünel sendromu, distal duyusal polinöropati, miyopati ve serebellar ataksi sıklıkla gözlenir. Hashimoto tiroiditi ile ilişkili olan ve nadir görülen Hashimoto ensefalopatisi (HE) ise tiroid hormon seviyelerinden bağımsız olarak akut veya subakut başlangıçlı kafa karışıklığı, bilinç değişiklikleri ve nöbetlerle seyreder. HE vakalarının büyük çoğunluğu kortikosteroid tedavisine dramatik ve olumlu bir yanıt verir. Genel olarak, her iki tiroid bozukluğunda da erken teşhis ve uygun tiroid hormon replasmanı ya da antitiroid tedaviler, nörolojik semptomların ve bilişsel işlevlerin geri döndürülmesinde kritik bir öneme sahiptir.

Thyroid dysfunctions are common endocrine disorders that significantly impact the peripheral and central nervous systems, leading to diverse neurological manifestations. Hyperthyroidism frequently causes cognitive impairments, memory deficits, high-frequency tremors, rare complications like chorea, and thyrotoxic myopathy. In severe cases such as thyroid storm, a life-threatening escalation can induce delirium, seizures, and coma. Conversely, hypothyroidism manifests with cognitive slowing, executive dysfunction, and short-term memory deficits, increasing the long-term risk of dementia. It is also closely associated with carpal tunnel syndrome, distal sensory polyneuropathy, cerebellar ataxia, and myopathy. A distinct, rare neuro-endocrine condition known as Hashimoto's encephalopathy (HE), linked to chronic autoimmune thyroiditis, presents with altered mental status, confusion, and seizures irrespective of the baseline thyroid hormone levels. HE typically shows a dramatic, favorable response to corticosteroid therapy. Ultimately, establishing an euthyroid state through timely antithyroid medications or levothyroxine replacement therapy is essential to reverse or mitigate most neurological and cognitive symptoms across the spectrum of thyroid diseases.
