Tiroid Fonksiyon Bozukluklarında Anestezi Yönetimi

Özet

Bu çalışma, tiroid fonksiyon bozukluğu (hipertiroidizm ve hipotiroidizm) olan hastaların anestezi yönetimini preoperatif, intraoperatif ve postoperatif dönemler çerçevesinde incelemektedir. Cerrahi stres altında tiroid hormon dengesizlikleri, anesteziyologlar için kritik ve yaşamı tehdit eden senaryolar oluşturabilmektedir. Elektif cerrahilerden önce hastaların farmakolojik tedaviyle mutlaka ötiroid duruma getirilmesi, kardiyovasküler ve solunumsal komplikasyon risklerini azaltmak açısından temel kuraldır. Hipertiroidi hastalarında en ciddi intraoperatif risk sempatik aşırı aktivite ve ameliyat sonrası %10-20 mortaliteye sahip tiroid fırtınasıdır; bu tablonun yönetiminde antitiroid ilaçlar, beta-blokerler ve steroidler kullanılır. Hipotiroidi vakalarında ise azalan kardiyak debi, baroreseptör refleks bozuklukları, alveolar hipoventilasyon, gecikmiş ilaç metabolizması ve perioperatif hipotermi eğilimi öne çıkmaktadır. Tedavi edilmemiş şiddetli hipotiroidi hastalarında enfeksiyon, cerrahi veya travma ile tetiklenebilen miksödem koması yüksek mortalite riski taşır ve acil intravenöz tiroid hormonu ile glukokortikoid replasmanı gerektirir. Her iki klinik durumda da büyük guatr veya kitlelerin varlığı zor havayolu yönetimini beraberinde getirdiği için ayrıntılı preoperatif radyolojik değerlendirme ve endotrakeal entübasyon tercih edilmelidir. Sonuç olarak, tiroid hastalarında başarılı bir anestezi yönetimi, perioperatif risklerin doğru analizine, yakın hemodinamik monitörizasyona ve multidisipliner destek tedavilerine bağlıdır.

This study examines the anesthetic management of patients with thyroid dysfunction (hyperthyroidism and hypothyroidism) across preoperative, intraoperative, and postoperative periods. Thyroid hormone imbalances under surgical stress create highly critical and life-threatening scenarios for anesthesiologists. Achieving a pharmacologically euthyroid state before elective surgery is a fundamental requirement to minimize cardiovascular and respiratory complications. In hyperthyroid patients, the most severe intraoperative risk is sympathetic overactivity and postoperative thyroid storm, which carries a 10-20% mortality rate; its management requires antithitroid drugs, beta-blockers, and steroids. Conversely, hypothyroid cases are characterized by decreased cardiac output, impaired baroreceptor reflexes, alveolar hypoventilation, delayed drug metabolism, and a high susceptibility to perioperative hypothermia. Untreated severe hypothyroidism can progress to myxedema coma, triggered by infection, surgery, or trauma, presenting a high mortality risk that mandates aggressive intravenous thyroid hormone and glucocorticoid replacement. In both clinical conditions, the presence of large goiters or masses introduces significant airway challenges, necessitating comprehensive preoperative radiological assessment and a preference for endotracheal intubation over laryngeal masks. Ultimately, successful anesthetic management in thyroid disorders relies heavily on precise perioperative risk analysis, close hemodynamic monitoring, and prompt multidisciplinary supportive care.
