Özet

Bu tıbbi metin, yüksek mortalite oranlarına sahip iki kritik endokrin acil durumu olan tiroid fırtınası ve miksödem komasını kapsamlı bir şekilde incelemektedir. Tiroid fırtınası, genellikle Graves hastalığı zemininde gelişen ve enfeksiyon, cerrahi veya ilaç düzensizliği gibi faktörlerle tetiklenen şiddetli bir tirotoksikoz tablosudur. Klinik olarak yüksek ateş, taşikardi, ajitasyon ve gastrointestinal disfonksiyon ile kendini gösteren bu durumun tanısı Burch ve Wartofsky skorlama sistemi gibi parametrelerle klinik olarak konur. Tedavisi laboratuvar sonuçları beklenmeden; destekleyici bakım, tiyonamidler, iyot, beta-blokörler ve periferik dönüşümü engelleyen glukokortikoidlerin kombinasyonunu içerir. Diğer taraftan miksödem koması, çoğunlukla yaşlı kadınlarda araya giren enfeksiyon veya ilaç kullanımıyla tetiklenen, ileri derece bir hipotiroidizm kliniğidir. Belirtileri arasında bilinç değişikliği, derin hipotermi, bradikardi, hiponatremi ve gode bırakmayan yaygın ödem yer alır. Miksödem komasının yönetiminde de acil klinik tanı hayati önem taşır; tedavi esas olarak agresif intravenöz T4 ve T3 hormon replasmanı ile eş zamanlı hidrokortizon uygulamasından oluşur. Her iki klinik tablo da yoğun bakım takibi gerektirmekte olup, erken ve agresif müdahale hayatta kalma oranlarını belirgin şekilde artırmaktadır.

Prompt clinical recognition is vital for myxedema coma; its treatment primarily revolves around aggressive intravenous T4 and T3 hormone replacement combined with hydrocortisone administration. Both conditions necessitate intensive care monitoring, and timely, aggressive therapeutic intervention significantly improves patient survival rates.
