Endokrin Bozukluklar ve İnfertilite İlişkisi

Özet

Sağlanan bilimsel metin, üreme sistemi ve endokrin bozukluklar arasındaki karmaşık ve doğrudan ilişkiyi incelemektedir. Hipofizer-Pitüiter-Over (HPO) döngüsü, ovulasyon ve uterus fonksiyonları açısından kritik öneme sahip olup; tiroid ve böbrek üstü bezleri gibi diğer endokrin organlardaki bozukluklar jinekolojik problemlere ve infertiliteye yol açmaktadır. Makalede, kadın infertilitesinin fonksiyonel hipotalamik amenore, aşırı egzersiz, stres ve beslenme eksikliği gibi hipotalamik nedenleri detaylandırılmaktadır. Ayrıca, üremeyi olumsuz etkileyen hipofiz bozukluklarından hiperprolaktinemi ve akromegali; adrenal bozukluklardan Cushing sendromu, konjenital adrenal hiperplazi (KAH) ve Addison hastalığı ele alınmaktadır. Üreme çağındaki kadınlarda sıkça görülen tiroid hastalıklarından hipotiroidizm ve hipertiroidizmin, hormonal dengeleri ve seks hormonu bağlayıcı globulin (SHBG) düzeylerini değiştirerek infertilite riskini artırdığı belirtilmektedir. Ovaryan fonksiyon bozuklukları kapsamında ise Polikistik Over Sendromu (PKOS) ve bunun neden olduğu anovulasyon ile insülin direnci süreçleri incelenmektedir. Sonuç olarak, infertilite şikayetiyle başvuran hastaların tanı sürecinde kapsamlı bir tıbbi geçmiş, fizik muayene ve prolaktin, TSH, FSH, E2 gibi laboratuvar testleriyle endokrin bozukluklar açısından bütüncül değerlendirilmesi gerektiği vurgulanmaktadır.

The provided scientific text examines the complex and direct relationship between the reproductive system and endocrine disorders. The Hypophysial-Pituitary-Ovarian (HPO) axis is of critical importance for ovulation and uterine functions; disorders in other endocrine organs, such as the thyroid and adrenal glands, lead to gynecological problems and infertility. The article details the hypothalamic causes of female infertility, such as functional hypothalamic amenore, excessive exercise, stress, and nutritional deficiencies. Furthermore, pituitary disorders that negatively affect reproduction, including hyperprolactinemia and acromegaly, as well as adrenal disorders like Cushing's syndrome, congenital adrenal hyperplasia (CAH), and Addison's disease, are discussed. It is stated that thyroid diseases, such as hypothyroidism and hyperthyroidism, which are frequently observed in women of reproductive age, increase the risk of infertility by altering hormonal balances and sex hormone-binding globulin (SHBG) levels. Within the scope of ovarian dysfunctions, Polycystic Ovary Syndrome (PCOS) and the associated processes of anovulation and insulin resistance are analyzed. In conclusion, it is emphasized that patients presenting with infertility complaints must be comprehensively evaluated for endocrine disorders during the diagnostic process through a detailed medical history, physical examination, and laboratory tests including prolactin, TSH, FSH, and E2 levels.
