Kulak Burun Boğazda Antimikrobiyaller ve Farmakoterapi
Özet
Kulak burun boğaz (KBB) uygulamalarında yaygın olarak tercih edilen antimikrobiyal ve farmakoterapötik ajanların sınıflandırılması, etki mekanizmaları, klinik endikasyonları ve potansiyel yan etkileri belgede kapsamlı bir şekilde incelenmektedir. KBB enfeksiyonlarında sıkça karşılaşılan gram pozitif, gram negatif, atipik ve anaerop bakterilere karşı uygulanan antibakteriyel tedaviler; penisilinler, sefalosporinler, karbapenemler, monobaktamlar, kinolonlar, makrolidler, aminoglikozitler, tetrasiklinler ve glikopeptidler gibi geniş bir yelpazede ele alınmaktadır. Ajanların etki spektrumları, sinerjistik kombinasyonları ve özellikle aminoglikozitlerin yol açtığı ototoksisite ile nefrotoksisite gibi kritik yan etkiler detaylandırılmaktadır. Antibakteriyel tedavilerin yanı sıra; invaziv fungal enfeksiyonlarda kullanılan sistemik ve topikal antifungaller, viral mukokutanöz lezyonlar ile influenza tedavisinde başvurulan antiviraller, antienflamatuar ve immünsupresif amaçlı glukokortikoidler ile steroid olmayan antienflamatuar ilaçlar (NSAİİ) etki güçleri ve risk faktörleriyle birlikte açıklanmaktadır. Son bölümde ise laringofaringeal reflü hastalığının medikal yönetiminde kullanılan asit nötralize edici, asit üretimini azaltıcı ve mide motilitesini artırıcı ilaçlar değerlendirilmektedir. KBB enfeksiyonlarındaki patojen çeşitliliği ve gelişen direnç mekanizmaları sebebiyle, klinisyenlerin güncel kılavuzlar ve laboratuvar verileri doğrultusunda doğru ajanı seçmesinin tedavi süreçlerinin yönetimindeki önemi vurgulanmaktadır.
The classification, mechanisms of action, clinical indications, and potential side effects of antimicrobial and pharmacotherapeutic agents commonly preferred in otorhinolaryngology (ENT) practices are comprehensively examined in the document.
