Akut Otitis Media ve Efüzyonlu Otitis Media
Özet
Akut otitis media (AOM), orta kulak boşluğunun enfeksiyon belirti ve bulgularıyla seyreden, özellikle ilk iki yaştaki çocuklarda sık görülen bir hastalığıdır. Östaki tüpünün gelişimiyle sıklığı azalan AOM; alerji, üst solunum yolu enfeksiyonları, sinüzit, sigara dumanı ve yatarak beslenme gibi nedenlerle tetiklenmektedir. Tedavisinde semptomların gerilemesi ve komplikasyonların önlenmesi amaçlanarak, hastanın yaşına ve hastalığın şiddetine göre ampirik antibiyoterapi (özellikle amoksisilin) veya gözlem tercih edilmektedir. Diğer taraftan, efüzyonlu otitis media (EOM), enfeksiyon bulguları olmaksızın orta kulak boşluğunda sıvı toplanmasıdır; 3 aydan uzun sürdüğünde kronikleşir. Östaki disfonksiyonu, enfeksiyon ve enflamasyon EOM oluşumunda rol oynayan üç ana faktördür. Tanısında pnömatik otoskopi, timpanogram ve odyogram kullanılırken; altın standart yöntem miringotomidir. Asemptomatik çocuklarda tedavi gerekmezken, medikal tedavide kortikosteroid, antibiyotik, dekonjestan veya antihistaminiklerin uzun süreli iyileşmede etkinliği saptanmamıştır. Belirli işitme kaybı, retraksiyon veya konuşma gecikmesi gibi durumlarda cerrahi olarak ventilasyon tüpü takılması ve yaşa göre adenoidektomi uygulanmaktadır. Her iki hastalık da zamanında tedavi edilmediğinde, mastoidit, fasial paralizi, labirentit gibi intratemporal veya menenjit, beyin absesi gibi ciddi intrakraniyal komplikasyonlara yol açabilmektedir.
Acute otitis media (AOM) is an infection of the middle ear cavity accompanied by signs and symptoms of inflammation, predominantly observed in children under two years of age. Triggered by factors like allergies, upper respiratory infections, sinusitis, secondhand smoke, and supine bottle-feeding, its incidence decreases as the Eustachian tube matures. Management strategies aim to relieve symptoms and prevent complications, utilizing empirical antibiotic therapy—primarily amoxicillin—or clinical observation based on the patient's age and clinical severity. Conversely, otitis media with effusion (OME) involves fluid accumulation in the middle ear without acute infection, classified as chronic if it persists for over three months. Eustachian tube dysfunction, infection, and inflammation represent the three primary etiologic factors. Diagnosis relies on pneumatic otoscopy, tympanometry, and audiometry, with myringotomy serving as the gold standard. Asymptomatic cases require no intervention; furthermore, corticosteroids, antibiotics, decongestants, and antihistamines lack long-term efficacy. Surgical intervention, involving ventilation tube insertion and age-appropriate adenoidectomy, is indicated for significant hearing loss, tympanic membrane retraction, or speech delays. If left untreated, both conditions can lead to severe intratemporal complications like acute mastoiditis and facial paralysis, or life-threatening intracranial complications such as meningitis and brain abscesses.
