Rinolojide Radyolojinin Yeri
Özet
Burun ve paranazal sinüslerin (PNS) değerlendirilmesinde konvansiyonel grafiler, bilgisayarlı tomografi (BT), manyetik rezonans görüntüleme (MR), ultrasonografi (USG) ve pozitron emisyon tomografisi (PET/BT) gibi çeşitli radyolojik yöntemler kullanılmaktadır. Düşük maliyetli konvansiyonel grafiler ilk basamakta tercih edilse de, düşük duyarlılık ve süperpozisyon gibi dezavantajları nedeniyle modern rinolojide rutin kullanımı önerilmemektedir. Günümüzde PNS görüntülemesinde altın standart olan BT; kemik yapıları, anatomik varyasyonları (nazal septum deviasyonu, konka bülloza vb.) ve cerrahi planlamayı en iyi gösteren kesitsel yöntemdir. Yumuşak doku çözünürlüğü en yüksek yöntem olan MR ise X-ışını içermez; enfeksiyon-tümör ayrımı, lezyonların çevre dokulara yayılımı ve tedavi sonrası nüks takibinde ön plana çıkar. USG'nin kullanımı cilt altı yumuşak doku ve kemik fraktürleri ile sınırlıyken, radyoaktif ajan (FDG) kullanılan PET/BT daha çok uzak metastaz saptanması ve evreleme amacıyla tercih edilir. Doğru radyolojik yöntem seçilirken maliyet, etkinlik, ulaşılabilirlik, hasta uyumu ve minimum radyasyon maruziyeti gibi etkenler göz önünde bulundurulmalıdır. Her ne kadar rinolojik hastalıkların çoğuna endoskopik ve klinik muayeneyle tanı konulabilse de, ihtiyaç halinde doğru endikasyonla seçilen radyolojik tetkikler hekimi en kısa sürede doğru tedaviye ulaştırmaktadır.
Various radiological modalities, including conventional radiography, computed tomography (CT), magnetic resonance imaging (MRI), ultrasonography (USG), and positron emission tomography (PET/CT), play a critical role in evaluating the nasal and paranasal sinus (PNS) diseases. Although conventional radiographs are initially preferred due to low cost, their routine use is not recommended in modern rhinology owing to low sensitivity and superposition disadvantages. Currently, CT is considered the gold standard for PNS imaging; it provides detailed cross-sectional visualization of bone structures, anatomical variations (such as nasal septum deviation and concha bullosa), and preoperative planning. MRI offers the highest soft-tissue contrast without radiation exposure, making it superior for differentiating inflammation from tumors, evaluating skull base lesions, and monitoring recurrence or residual disease post-treatment. While USG utilization remains limited to subcutaneous soft tissue and certain bone fractures, PET/CT utilizing radioactive tracers (FDG) is primarily indicated for staging and detecting distant metastases. Selecting the optimal imaging modality requires careful consideration of cost, efficacy, accessibility, patient compliance, and minimizing radiation exposure. Although most rhinolologic conditions are diagnosed via clinical and endoscopic examinations, appropriate radiological imaging selected with the correct indication significantly accelerates accurate diagnosis and treatment planning.
