Obstrüktif Uyku Apne Sendromu
Özet
Obstrüktif Uyku Apne Sendromu (OSAS), uyku sırasında üst solunum yolunun tekrarlayan tam veya kısmi tıkanmaları ve oksijen satürasyonunun düşmesiyle karakterize, ciddi morbidite ve mortaliteye yol açan yaygın bir hastalıktır. Obezite ve erkek cinsiyet en önemli risk faktörleri olup, hastalık en sık 40-65 yaş grubunda görülür. En belirgin semptomları horlama, gündüz aşırı uyku hali ve yorgun uyanmadır. Tanıda altın standart, en az 6 saat kesintisiz veri kaydeden polisomnografidir (PSG). PSG eşliğinde hesaplanan Apne Hipopne İndeksi (AHİ) ≥5 olan ve komorbiditeleri bulunan hastalara tedavi planlanır. Ayrıca sefalometri, nazal endoskopik muayene ve Müller manevrası gibi yardımcı tanı yöntemleri de kullanılır. Tedavide en sık uygulanan ve üst hava yolunu açık tutan yöntem pozitif havayolu basıncı (PAP) uygulamasıdır; hastanın durumuna göre CPAP, APAP, BPAP veya BPAP-ST cihazları seçilir. Alternatif olarak, hava yolundaki tıkanıklığı ve direnci azaltmak amacıyla septoplasti, uvulopalatofaringoplasti ve dil kökü süspansiyonu gibi cerrahi işlemler uygulanabilir. Cerrahide başarı, postoperatif PSG'de AHİ'nin 20'nin altına ya da preoperatif dönemin en az yarısına indirilmesiyle ölçülür.
Obstructive Sleep Apnea Syndrome (OSAS) is a common disorder characterized by recurrent complete or partial obstructions of the upper airway during sleep and a drop in oxygen saturation, leading to significant morbidity and mortality. Obesity and male gender are the primary risk factors, with the highest prevalence observed in the 40-65 age group. The most prominent symptoms include snoring, excessive daytime sleepiness, and waking up tired. Polysomnography (PSG), requiring a minimum of 6 hours of continuous recording, remains the gold standard for diagnosis. Treatment is indicated for patients with an Apnea-Hypopnea Index (AHI) ≥5, evaluated alongside comorbidities. Supplementary diagnostic tools include cephalometry, nasal endoscopic examination, and the Müller maneuver. The most common therapeutic approach is Positive Airway Pressure (PAP) therapy, which keeps the airway open using devices like CPAP, APAP, BPAP, or BPAP-ST based on patient needs. Alternatively, surgical interventions such as septoplasty, uvulopalatopharyngoplasty, and tongue base suspension aim to eliminate airway obstruction and resistance. Surgical success is objectively defined as reducing the postoperative AHI below 20 or to at least half of its preoperative value.
