Özet
Larinksin (gırtlak) anatomisi, kıkırdakları, kasları, sinirsel beslenmesi ile histolojik yapısını detaylandıran bu çalışma; organı etkileyen konjenital, edinsel, benign, enfeksiyöz ve sistemik hastalıkları kapsamlı şekilde incelemektedir. Larinksin embriyolojik gelişimi ve anatomik sınırları aktarıldıktan sonra, vokal kordların beş tabakadan oluşan histolojik dokusu sunulmaktadır. Çocukluk çağında stridora yol açan laringomalazi, subglottik stenoz, konjenital vokal kord paralizileri, laringeal atrezi, hemanjiom, web ve yarık gibi doğumsal anomalilerin tanı, evreleme ve tedavi protokolleri açıklanmaktadır. Erişkinlerde karşılaşılan laringosel, sulkus vokalis ve tek ya da çift taraflı vokal kord felçleri gibi klinik tabloların yanında; akut larinjit, epigloit ve krup gibi acil müdahale gerektirebilecek enfeksiyöz süreçler ele alınmaktadır. Ayrıca sigara kullanımı ve sesin kötüye kullanımıyla ilişkili olan Reinke ödemi, vokal kord polipleri, nodülleri ile kontakt granülomlar ve kistler gibi iyi huylu lezyonların fono-cerrahi, ses terapisi ve medikal yaklaşımları özetlenmektedir. Son olarak amiloidoz, romatoid artrit, sarkoidoz ve Wegener granülomatozu gibi sistemik romatolojik ve vaskülitik patolojilerin larinksteki tutulum biçimleri, yarattıkları havayolu obstruksiyonu riskleri ve tedavi seçenekleri aktarılmaktadır.
Detailing the anatomy, cartilages, muscles, neurovascular supply, and histological structure of the larynx, this comprehensive study examines the congenital, acquired, benign, infectious, and systemic diseases affecting the organ. Following the description of embryological development and anatomical boundaries, the five-layer histological structure of the vocal folds is presented. Diagnosis, staging, and treatment protocols for congenital anomalies causing stridor in infants, such as laryngomalacia, subglottic stenosis, vocal cord paralysis, laryngeal atrezi, hemangioma, web, and clefts, are fully explained. Clinical conditions encountered in adults, including laryngocele, sulcus vocalis, and unilateral or bilateral vocal cord immobility, are discussed alongside infectious processes like acute laryngitis, epiglottitis, and croup that may require urgent intervention. Furthermore, management strategies, including phonosurgery, voice therapy, and medical care, are outlined for benign lesions such as Reinke's edema, vocal cord polyps, nodules, cysts, and contact granulomas linked to smoking and vocal abuse. Finally, the laryngeal manifestations of systemic diseases like amyloidosis, rheumatoid arthritis, sarcoidosis, and Wegener's granulomatosis are reviewed, highlighting airway obstruction risks and specialized treatment approaches.
