Vokal Kord Hastalıkları Mikrocerrahisi
Özet
Vokal kord hastalıkları mikrocerrahisi, sesin iyileştirilmesi ve restorasyonunu hedefleyen fonocerrahi yöntemlerinin önemli bir parçasıdır. Bu cerrahi yaklaşım, ses kalitesini ve yaşam kalitesini artırmayı temel amaç edinir. Cerrahi öncesinde hastanın mesleği, yaşı, beklentileri, geçirdiği travmalar ve sigara/alkol kullanımı detaylıca sorgulanmalıdır. Sesin ve kord hareketlerinin değerlendirilmesinde rijid/fleksible endoskoplar ile altın standart kabul edilen videolarengostroboskopiden yararlanılır. Müdahale kararı verilmeden önce medikal tedaviler ve ses terapileri gibi cerrahi dışı alternatifler mutlaka göz önünde bulundurulmalıdır. Mikrocerrahinin temel prensibi, epitel ve Reinke tabakasını koruyarak doku hasarını ve cerrahi travmayı minimal düzeyde tutmaktır. Subepitelyal polip, nodül ve kist gibi iyi huylu lezyonların tedavisinde en sık tercih edilen soğuk diseksiyon yöntemi "mikroflep" tekniğidir. Papillomatozis ve vasküler ektazilerde ise lazer teknolojilerinden faydalanılır. Ameliyat sonrasında boğaz ağrısı, dilde uyuşma, öksürük ve tat bozukluğu gibi geçici komplikasyonlar görülebilir. Profilaktik antibiyotik kullanımı önerilmezken, iyileşme sürecinde kişiye özel ses istirahati, videostroboskopik kontroller, ses terapileri ve reflü diyeti gibi davranışsal önlemler büyük önem taşır.
Microsurgery for vocal cord diseases is a vital component of phonosurgery, specifically aimed at improving and restoring voice quality. The primary goal of this surgical intervention is to enhance the patient's quality of life. Comprehensive preoperative assessments involve detailed questioning regarding the patient's age, profession, expectations, past laryngeal traumas, and smoking or alcohol habits. For visualization, rigid or flexible endoscopes and the gold-standard videolaryngostroboscopy are utilized to evaluate vocal cord vibrations. Before deciding on surgery, non-surgical alternatives like medical treatments and voice therapy must be thoroughly evaluated. The fundamental principle of phonomicro surgery is to minimize surgical trauma and tissue damage by preserving the epithelium and Reinke's space. The microflap technique via cold dissection is the most preferred method for benign subepithelial lesions such as polyps, nodules, and cysts. Additionally, laser technologies are leveraged for conditions like papillomatosis and vascular ectasias. Postoperative complications may include temporary throat pain, tongue numbness, coughing, and taste disturbances. Prophylactic antibiotics are not recommended due to low infection risks. Postoperative care relies heavily on tailored voice rest, videostroboscopic follow-ups, voice therapy, and anti-reflux dietary modifications to optimize long-term vocal recovery.
