Postpartum Dönemde Tromboembolik Olaylar
Özet
Gebelikte ve postpartum dönemde venöz tromboemboli (VTE) riski, sağlıklı kadınlarda normal dönemlere kıyasla 4-5 kat artmaktadır. Gelişmiş ülkelerde VTE görülme oranı binde bir iken, ileri yaşlarda bu oran yüzde bire kadar yükselebilmektedir. VTE olaylarının yaklaşık %75-80'i derin ven trombozu (DVT), %20-25'i ise pulmoner emboli kaynaklıdır ve bu vakaların yaklaşık yarısı doğum sonrası süreçte meydana gelmektedir. Trombüs oluşumunun etyolojisinde venöz staz, endotel hasarı ve hiperkoagülasyondan oluşan Virchow üçlüsü kritik rol oynamaktadır. Postpartum dönemde VTE gelişimindeki en önemli risk faktörü geçirilmiş VTE öyküsüdür; bunun yanı sıra sezaryen doğum, 35 yaş üstü olmak, obezite (VKİ ≥30 kg/m²), uzamış yatak istirahati, preeklampsi, sigara kullanımı ve postpartum enfeksiyonlar risk derecesini ciddi şekilde artırmaktadır. Sağlık Bakanlığı ve RCOG gibi kuruluşlar, her gebenin erken dönemde ve hastaneye yatışlarda VTE risk skorlamasıyla değerlendirilmesini önermektedir. VTE profilaksisinde erken mobilizasyon, hidrasyonun sağlanması, elastik kompresyon çorapları kullanımı ve hastanın risk durumuna göre düşük molekül ağırlıklı heparin (DMAH) veya warfarin gibi farmakolojik ajanların lohusalıkta en az 10 gün ile 6 hafta arasında uygulanması ve bu süreçte ebe ile hemşirelerin hasta eğitiminde aktif rol oynaması hayati önem taşımaktadır.
The risk of venous thromboembolism (VTE) during pregnancy and the postpartum period increases 4-5 times in healthy women compared to other periods of life. While the incidence of VTE is 1/1000 in developed countries, it can rise up to 1/100 at older ages. Approximately 75-80% of VTE events originate from deep vein thrombosis (DVT) and 20-25% from pulmonary embolism, with about half of these cases occurring during the postpartum stage. The Virchow triad, consisting of venous stasis, endothelial injury, and hypercoagulability, plays a critical role in the etiology of thrombus formation. The most significant risk factor for VTE in the postpartum period is a history of previous VTE; additionally, cesarean delivery, age over 35, obesity (BMI ≥30 kg/m²), prolonged bed rest, preeclampsia, smoking, and postpartum infections substantially elevate the risk level. Organizations such as the Ministry of Health and RCOG recommend evaluating every pregnant woman with VTE risk scoring during early pregnancy and hospital admissions. In VTE prophylaxis, early mobilization, ensuring hydration, using elastic compression stockings, and administering pharmacological agents like low-molecular-weight heparin (LMWH) or warfarin for 10 days to 6 weeks based on the patient's risk profile are vital, with midwives and nurses playing an active role in patient education.
