Özet

Doğum sonrası (postpartum) dönem, kadınları psikiyatrik bozukluklara karşı savunmasız hale getiren ve literatürde genel olarak annelik hüznü, lohusa psikozu ve doğum sonrası depresyon (PPD) olarak sınıflandırılan geniş bir fenomendir. En sık görülen ve ilaç tedavisi gerektirmeyen annelik hüznü, doğumdan sonraki ilk günlerde geçici duygudurum dalgalanmalarıyla ortaya çıkar ve genellikle iki hafta içinde kendiliğinden düzelir. Doğum sonrası depresyon (PPD) ise annelerin %10-15'ini etkileyen, tedavi edilmediğinde kronikleşebilen ve anne-bebek bağlanmasını, bebeğin gelişimini ve tüm aile dinamiklerini olumsuz etkileyen ciddi bir duygudurum bozukluğudur. PPD'nin etiyolojisinde psikiyatrik öykü, hormonal dalgalanmalar, obstetrik komplikasyonlar, yetersiz sosyal destek ve uyku düzensizlikleri gibi çok boyutlu risk faktörleri rol oynar. Tedavisinde ilk basamak olarak bilişsel davranışçı terapiler gibi psikososyal müdahaleler uygulanırken, dirençli vakalarda seçici serotonin geri alım inhibitörleri (SSRI) ve elektrokonvülsif tedavi (EKT) tercih edilir. Doğum sonrası psikoz ise sanrılar, halüsinasyonlar ve hem anne hem bebek için hayati riskler barındıran acil bir psikiyatrik tablodur ve hastanede yatarak tedavi gerektirir. Bu süreçte, ebeler, hemşireler ve birinci basamak sağlık çalışanları, prenatal ve postnatal dönemlerdeki yakın izlemleriyle semptomların erken teşhisinde, riskli grupların takibinde, aile eğitiminde ve uygun uzmana yönlendirmede kilit rol oynarlar.

The postpartum period is a complex phase that renders women highly vulnerable to psychiatric disorders, which are traditionally categorized into maternity blues, postpartum psychosis, and postpartum depression (PPD). Maternity blues, the most prevalent and self-limiting condition, manifests as transient mood fluctuations during the early days following childbirth and typically resolves spontaneously within two weeks without requiring pharmacological intervention. Postpartum depression, affecting approximately 10-15% of new mothers, is a severe mood disorder that can become chronic if left untreated, adversely impacting mother-infant attachment, child development, and overall family dynamics. The etiology of PPD involves multidimensional risk factors, including a history of psychiatric illness, hormonal fluctuations, obstetric complications, inadequate social support, and sleep deprivation. While psychosocial interventions like cognitive behavioral therapy serve as the primary line of treatment, selective serotonin reuptake inhibitors (SSRIs) and electroconvulsive therapy (ECT) are preferred in severe or resistant cases. Postpartum psychosis represents a psychiatric emergency characterized by delusions and hallucinations, posing significant risks of harm to both the mother and the infant, thereby necessitating immediate hospitalization. Throughout this critical timeline, midwives, nurses, and primary healthcare professionals play a pivotal role through close prenatal and postnatal monitoring, enabling early detection, managing high-risk groups, providing family education, and ensuring timely psychiatric referrals.
