Özet

Deri, insan vücudunu dış etkenlerden koruyan üç katmanlı hayati bir organdır ve bu dokunun bütünlüğünün hasar görmesiyle akut veya kronik yaralar meydana gelir. Yarayı mikroorganizmalardan korumak, sızıntı ile sıvıları emmek, kanamayı durdurmak ve iyileşme sürecini hızlandırmak amacıyla steril malzemelerle uygulanan yara bakım işlemine pansuman adı verilmektedir. Tarihsel gelişiminde Mezopotamya’da kil tabletler ve bal, Antik Yunan’da sirke kullanılırken, 19. yüzyılda antibiyotikler ve gazlı bezler devreye girmiş, 20. yüzyılda ise nemli ortam sunan modern yara pansumanları kullanılmaya başlanmıştır. İdeal bir pansuman malzemesi nem dengesini korumalı, gaz alışverişine izin vermeli, dokuya yapışmamalı, toksik olmamalı, steril ve ekonomik özellikler taşımalıdır. Yara bakımında doğru pansumanın seçilebilmesi için yaranın türü, derinliği, akıntı miktarı ve enfeksiyon varlığı gibi kriterler yönünden nekrotik, kabuklu, granülasyonlu veya epitelize sınıflarına göre sürekli olarak değerlendirilmesi ve klinik öykü alınması gerekir. Sınıflandırma bakımından pansumanlar yaranın durumuna göre koruyucu, emici ve basınçlı; yapılış şekline göre ise kuru, ıslak, garrel ve hazır steril pansuman olarak ayrılır. Geleneksel yöntemler hafif eksüdalı yaraları sadece korurken, modern pansumanlar dehidratasyonu önleyip bakteri bariyeri oluşturarak iyileşmeyi destekler. Hemşirelik bakımında ise aseptik tekniklere uyulması ve yaranın takibi sürecin başarısını belirler.

The skin is a vital three-layered organ that protects the human body, and disruptions in its epithelial continuity due to physical or thermal damage result in acute or chronic wounds. Dressing refers to the sterile wound care procedure designed to protect wounds from microorganisms, absorb exudate, prevent pain, and accelerate the healing process. Historically, wound treatment evolved from Mesopotamian clay tablets and Ancient Greek wine or vinegar to 19th-century antibiotics and gauze, culminating in the 20th-century development of modern moisture-retaining dressings. An ideal dressing material must maintain a moist environment, permit gas exchange, prevent bacterial infection, remain non-toxic, and be sterile and cost-effective. Selecting the appropriate dressing requires a thorough clinical history and continuous assessment of the wound bed based on depth, exudate, and appearance, classifying wounds into necrotic, sloughy, granulating, or epithelializing categories. Dressings are classified by wound status into protective, absorbent, and pressure dressings, and by application characteristics into dry, wet, garrel, and ready-to-use sterile dressings. Unlike traditional dressings that merely cover clean wounds and absorb fluid, modern dressings actively prevent dehydration and serve as a bioactive barrier against bacteria. Finally, nursing care plays a critical role, necessitating strict adherence to surgical aseptic techniques, monitoring vital signs, and ensuring continuous wound evaluation for effective treatment.
