Ateşli Silah Yaralanmaları
Özet
Ateşli silah yaralanmaları, hızlı ve doğru müdahale gerektiren, dünya genelinde mortalite oranı yüksek ciddi bir halk sağlığı sorunudur. Her yıl binlerce kişinin etkilendiği bu vakalarda ölümler sıklıkla cinayet, intihar ve kazalar sonucu meydana gelmektedir. Yaralanmalar sonrasında periferik sinir, damar ve kas hasarları, enfeksiyonlar, kompartman sendromu ve kemik kaynamama sorunları gibi biyolojik ve mekanik komplikasyonlar gelişebilmektedir. Tedavi süreçleri; ilk muayenede mermi giriş-çıkış deliklerinin ve ikincil travmaların değerlendirilmesi, antibiyotik ve tetanoz profilaksisi, debridman, irrigasyon ve gereken durumlarda cerrahi stabilizasyon ile resüsitasyon uygulamalarını kapsamaktadır. Bu süreçte klinisyen, iletişimci ve işbirlikçi rolleriyle kilit öneme sahip olan hemşireler, hastaya bütüncül bir bakım sunarken aynı zamanda tıbbi müdahaleyi geciktirmeden kıyafet, mermi kovanı gibi adli delillerin korunması ve resmi tutanakla yetkililere teslim edilmesi sorumluluğunu üstlenirler.
Gunshot wounds are a critical public health issue worldwide with a high mortality rate, requiring rapid and accurate intervention. Affecting thousands of people annually, these fatalities frequently occur as a result of homicide, suicide, and accidents. Following injuries, biological and mechanical complications may develop, including peripheral nerve, vascular, and muscle damage, infections, compartment syndrome, and bone nonunion. Treatment processes encompass the evaluation of bullet entry-exit wounds and secondary traumas during the initial examination, antibiotic and tetanus prophylaxis, debris removal, irrigation, and when necessary, surgical stabilization and resuscitation. Nurses, holding key importance with their clinical, communicative, and collaborative roles in this period, provide holistic care to the patient while simultaneously undertaking the responsibility of preserving forensic evidence such as clothing or bullet casings without delaying medical intervention, and delivering them to authorities via official records.
