Paraneoplastik Sendromların Tanım ve Sınıflandırılması
Özet
Paraneoplastik sendromlar (PS), bir tümörün veya metastazlarının doğrudan fiziksel etkileriyle ilişkili olmayan, malign kitle uzakta olsa bile varlığına bağlı olarak gelişen ve tümörün tedavisiyle gerileyebilen belirtiler bütünüdür. Kanser hastalarının yaklaşık %7-10'unda tanı anında, %50'sinde ise hastalık sürecinde görülen bu sendromlar; en sık küçük hücreli akciğer kanseri, meme, jinekolojik tümörler ve hematolojik malignitelerle ilişkilidir. Patogenezi tam olarak çözülememiş olsa da en kabul gören mekanizmalar; tümör hücrelerinin hormon, peptit, sitokin gibi biyolojik aktif bileşenleri aşırı sentezlemesi veya bağışıklık sisteminin ürettiği antikorların sağlıklı dokularla çapraz reaksiyona girmesidir. PS'ler endokrin, nörolojik, dermatolojik, nefrolojik, romatolojik ve hematolojik sistemleri etkileyebilir. Çoğu zaman primer tümör teşhis edilmeden önce ortaya çıktıkları için kanserin erken evrede saptanmasında ve küratif tedavi şansının artırılmasında kritik bir öneme sahiptirler. Ancak, metastatik hastalıkla karıştırılma riskleri yüksek olduğundan, detaylı fizik muayene ve ek tetkiklerle ayırıcı tanı yapılması hayati önem taşır. Nadir görülmeleri ve teşhis zorlukları nedeniyle prospektif çalışmalar sınırlı olup, hücre biyolojisindeki gelişmelerle mortaliteyi azaltacak yeni tedavi seçeneklerinin geliştirilmesi hedeflenmektedir.
Paraneoplastic syndromes (PS) are a complex group of clinical signs and symptoms that are not directly related to the local or metastatic physical effects of a tumor, yet occur at sites distant from the primary lesion due to its presence. These syndromes affect approximately 7-10% of cancer patients at the time of diagnosis and up to 50% during the course of the disease, being most commonly associated with small cell lung cancer, breast, gynecological, and hematological malignancies. Although the exact pathogenesis remains fully unelucidated, the most widely accepted mechanisms involve the tumor’s hypersecretion of biologically active components such as hormones, peptides, and cytokines, or an immune response where tumor-induced antibodies cross-react with healthy tissues. PS can affect multiple organ systems, leading to endocrine, neurological, dermatological, nephrological, rheumatological, and hematological manifestations. Because paraneoplastic disorders often manifest before a primary cancer is identified, recognizing them early is crucial for detecting tumors in their early, highly treatable stages, thereby increasing curative outcomes. However, since they frequently mimic metastatic disease, careful differential diagnosis via thorough physical examinations and diagnostic tests is vital. Prospective studies remain limited due to their rarity, and further cellular biology research is needed to develop effective therapies targeting mortality-inducing syndromes.
