Özet

Polisitemi, periferik kanda hemoglobin veya hematokrit konsantrasyonunun artması durumudur. Bu durum, plazma hacminin azalmasından kaynaklanan rölatif bir etki olabileceği gibi, kemik iliğindeki genetik veya kazanılmış mutasyonlara bağlı primer bir artıştan veya artmış eritropoetin (EPO) seviyelerine sekonder olarak da gelişebilir. Oldukça nadir görülen paraneoplastik polisitemiler ise bazı tümörlerin oksijenden bağımsız olarak otonom EPO üretmesi sonucu ortaya çıkan sekonder bir polisitemi türüdür. Bu antiteye en sık renal hücreli karsinom, hepatosellüler karsinom, hemanjioblastom, feokromasitoma ve uterin miyomlar neden olmaktadır. Tanı aşamasında tam kan sayımı, serum EPO düzeyi ve JAK2 mutasyon analizinin yanı sıra, altta yatan primer tümörü tespit etmek amacıyla toraks, tüm abdomen ve beyin görüntüleme yöntemlerinden yararlanılır. Paraneoplastik polisiteminin klinik yönetimi, altta yatan primer kanserin tedavisine odaklanmaktadır ve bu doğrultuda temel yaklaşım, EPO salgılayan tümörün cerrahi olarak çıkarılmasıdır. Lokalize hastalıklarda kesin tedavi nefrektomi gibi cerrahi yöntemlerle sağlanırken, başarılı bir tedavi sonrasında polisiteminin nüksetmesi tümörün geri döndüğüne işaret edebileceğinden, hastaların takibinde polisitemi varlığı kritik bir biyobelirteç rolü üstlenir.

Polycythemia is defined as an increase in the concentration of hemoglobin and/or hematocrit in the peripheral blood. This condition can manifest as relative polycythemia due to a decrease in plasma volume, primary polycythemia caused by genetic or acquired mutations in bone marrow progenitor cells, or secondary polycythemia resulting from elevated erythropoietin (EPO) levels. Paraneoplastik polycythemias are a rare form of secondary polycythemia triggered by the autonomous, oxygen-independent production of EPO by certain malignant or benign tumors. The most frequently associated neoplasms include renal cell carcinoma, hepatocellular carcinoma, hemangioblastoma, pheochromocytoma, and uterine leiomyomas. Diagnostic evaluation encompasses complete blood count, serum EPO level measurement, and JAK2 mutation testing, alongside extensive imaging modalities such as thoracic, abdominal, and brain scans to locate the underlying tumor. The primary clinical management strategy focuses on treating the underlying malignancy, with surgical resection of the tumor serving as the definitive therapeutic intervention. In localized diseases, such as renal cell carcinoma, while the primary treatment is nephrectomy, monitoring the recurrence of polycythemia post-surgery is crucial, as its re-emergence strongly suggests tumor recurrence or metastasis.
