Özet

Paraneoplastik alerjik reaksiyonlar, bağışıklık sisteminin kanser hücrelerinin varlığına veya salgıladığı humoral faktörlere karşı geliştirdiği, doğrudan tümör yayılımı ya da tedavi yan etkilerine bağlı olmayan sistemik yanıtlardır. Alerji ve onkoloji arasındaki bu karmaşık ilişki; inatçı kaşıntı, kronik ürtiker, edinilmiş anjioödem, ürtikeryal vaskülit ve hipereozinofili gibi klasik alerjik semptomlar şeklinde klinik olarak ortaya çıkabilmektedir. Bu semptomlar genellikle geleneksel antihistaminik ve steroid tedavilerine dirençli olup, altta yatan malignitenin tespiti öncesinde, eş zamanlı olarak veya bir nüks belirtisi şeklinde kendini gösterebilir. Hodgkin lenfoma, lösemi gibi hematolojik neoplazmların yanı sıra meme, akciğer ve yumurtalık kanseri gibi solid tümörler de bu reaksiyonları tetikleyebilmektedir. Tanısal süreçte, semptomların standart alerji testleriyle açıklanamaması durumunda kişiye özel kanser taramalarının yapılması, erken teşhis ve tedavi başarısı için kritik öneme sahiptir. Paraneoplastik kökenli bu tablolarda, primer tümörün cerrahi olarak çıkarılması veya sistemik onkolojik tedavilerin uygulanmasıyla humoral uyarıcılar ortadan kalkmakta ve alerjik semptomlar genellikle birkaç gün içinde kendiliğinden tamamen gerilemektedir.

Paraneoplastic allergic reactions are systemic immune responses triggered by the presence of cancer cells or tumor-derived humoral factors, rather than direct tumor invasion or treatment side effects. This intricate relationship between allergy and oncology manifests through classic allergic symptoms such as intractable pruritus, chronic urticaria, acquired angioedema, urticarial vasculitis, and hypereosinophilia. These symptoms are typically resistant to standard antihistamine and steroid therapies and can appear months or years before the diagnosis of the underlying malignancy, concurrently with it, or as a warning sign of tumor recurrence. Hematological neoplasms like Hodgkin lymphoma and leukemia, as well as solid tumors including breast, lung, and ovarian cancers, are commonly associated with these phenomena. In clinical practice, when conventional etiologies fail to explain chronic symptoms, age-appropriate cancer screening becomes vital for early diagnosis and improved patient survival. The definitive management of these paraneoplastic manifestations relies on treating the underlying malignancy; surgical resection or systemic oncological therapy eliminates the source of humoral triggers, leading to the rapid and complete resolution of allergic symptoms.
