Özet

Ses problemlerinin değerlendirilmesi, hastanın tıbbi geçmişinin ve kulak burun boğaz muayenesinin detaylı incelenmesiyle başlayan, objektif ve subjektif yöntemleri içeren kapsamlı bir süreçtir. Objektif değerlendirmede; larenks yapılarının ve ses tellerinin vibrasyon hareketlerinin yavaşlatılarak izlenmesini sağlayan laringeal ayna, rijid veya fleksibıl endoskopi ile videolaringostroboskopi gibi görüntüleme teknikleri kritik rol oynar. Ayrıca yüksek hızda dijital görüntüleme, videokimografi ve elektroglottografi gibi ileri yöntemlerle vokal kord hareketleri detaylandırılır. Akciğer kapasitesini ve ses tellerinin kapanma yeteneğini ölçen aerodinamik analizlerde vital kapasite, maksimum fonasyon zamanı ve s/z oranı gibi parametreler incelenirken; akustik analiz programlarıyla temel frekans, jitter, shimmer ve gürültü-harmonik oranları sayısal verilere dönüştürülür. Subjektif değerlendirme ise hastanın ve klinisyenin kişisel görüşlerine dayanır. Hastalar Ses Handikap Endeksi, Sesle İlgili Yaşam Kalitesi Ölçeği ve Ses Yorgunluk Ölçeği gibi anketlerle yaşadıkları problemin fizyolojik, psikolojik ve sosyal etkilerini puanlarlar. Klinisyenler ise işitsel algısal değerlendirme sürecinde sesin kısıklık, pürüzlülük, nefeslilik, rezonans gibi özelliklerini GRBAS, RBH veya CAPE-V gibi standardize edilmiş uluslararası skalalar ve protokoller kullanarak derecelendirirler. Tüm bu objektif ve subjektif yöntemlerin birlikte kullanımı, ses hastalıklarının doğru teşhisinde, kişiye özel tedavi planlamasında ve cerrahi ile terapi sonuçlarının takibinde güvenilir bir temel oluşturur.

The evaluation of voice disorders is a comprehensive process integrating objective and subjective methods, beginning with a detailed patient history and an otorhinolaryngological examination. In objective evaluation, imaging techniques such as laryngeal mirror, rigid or flexible endoscopy, and videolaryngostroboscopy play a critical role by allowing the visualization of vocal fold vibrations in slowed motion. Furthermore, advanced methods like high-speed digital imaging, videokymography, and electroglottography provide detailed insights into vocal cord dynamics. Aerodynamic analysis measures lung capacity and vocal fold closure using parameters like vital capacity, maximum phonation time, and the s/z ratio, while acoustic analysis software converts voice waves into quantitative data including fundamental frequency, jitter, shimmer, and noise-to-harmonic ratios. Subjective evaluation relies on the personal perspectives of both patients and clinicians. Patients assess the physiological, psychological, and social impacts of their condition using questionnaires such as the Voice Handicap Index, Voice-Related Quality of Life, and Vocal Fatigue Index. Clinicians perform auditory-perceptual assessments, rating voice characteristics like roughness, breathiness, and strain using standardized international scales and protocols such as GRBAS, RBH, or CAPE-V. The combined utilization of these objective and subjective methods establishes a reliable foundation for accurate diagnosis, customized treatment planning, and monitoring therapeutic outcomes.
