Özet

Fonocerrahi, ses kalitesini artırmak veya değiştirmek amacıyla laringoskop, endoskop ya da mikroskop kullanılarak genel anestezi altında gerçekleştirilen fonksiyonel cerrahiler bütünüdür. Başarılı cerrahi sonuçlar elde etmek için vokal kordların çok katmanlı mikroanatomik yapısına (epitel, lamina propria katmanları ve vokalis kası) saygı duyulması ve doku bütünlüğünün korunması esastır. Cerrahi planlama öncesinde stroboskopik değerlendirme altın standart kabul edilirken, hastanın ilaç kullanımı, menstruasyon dönemi ve akut fonotravma durumları titizlikle sorgulanmalıdır. Operasyon sırasında hava yolu yönetimi için küçük endotrakeal tüpler, jet ventilasyon veya apneik yöntemler tercih edilebilir. Temel fonomikrocerrahi aletlerinin (makas, forseps, elevatör) yanı sıra yüksek görüntü kalitesine sahip mikroskoplar ve CO2 lazer teknolojisi kullanılmaktadır. Fonksiyonel iyileşmeyi maksimuma çıkarmak ve skarları önlemek amacıyla, lezyonun vokal ligamana yapışıklık derecesine göre medial veya lateral mikroflep teknikleri uygulanır. Cerrahi sonrasında doku iyileşmesini desteklemek için klinikte en az 5 günlük mutlak ses istirahati, proton pompa inhibitörleri ve birinci aydan itibaren ses terapisi önerilmektedir.

Phonosurgery encompasses functional surgical procedures performed under general anesthesia using laryngoscopes, endoscopes, or microscopes to improve or alter voice quality. Achieving successful surgical outcomes relies heavily on respecting the multilayered microanatomical structure of the vocal folds—including the epithelium, lamina propria layers, and vocalis muscle—while preserving tissue integrity. Prior to surgery, stroboscopic evaluation serves as the gold standard for diagnosis, and patient factors such as medication use, menstruation, and acute phonotrauma must be carefully evaluated. Airway management during the procedure utilizes small endotrakeal tubes, jet ventilation, or apneic techniques. The primary surgical toolkit includes microinstruments like scissors, forceps, and elevators, complemented by high-resolution microscopes and CO2 laser systems. To optimize functional recovery and prevent scarring, surgeons utilize either medial or lateral microflap techniques based on the lesion's adherence to the vocal ligament. Postoperatively, to support optimal tissue healing, the clinical protocol recommends a minimum of 5 days of absolute voice rest, proton pump inhibitors to prevent hyperacidity, and the initiation of formal voice therapy starting from the first month.
