Disfaji ve Yutma Bozukluğu Rehabilitasyonu

Özet

Yutma bozukluklarının rehabilitasyonu (YBR), yutmanın tüm fazlarında yaşanan sorunların aşılması ve aspirasyon riskinin ortadan kaldırılması amacıyla yapılan multidisipliner tedavi yaklaşımları bütünüdür. YBR ünitesinde FTR veya KBB uzmanı liderliğinde dil ve konuşma terapisti, hemşire ve diyetisyenden oluşan uyumlu bir ekip çalışması gerekir. Yetişkinlerde inme, Parkinson, baş-boyun maligniteleri ve cerrahiler disfaji etiyolojisinde ilk sıralarda yer alırken; gençlerde sistemik otoimmün ve konjenital hastalıklar ön plana çıkar. Tedavi seçenekleri arasında ağız açıklığı ve droolinge yönelik egzersizler, dil güçlendirme, duyusal taktil stimülasyon, boyun postüral manevraları ve egzersizleri yer alır. Ayrıca baş önde yutkunma, eforlu yutkunma, larinks mobilizasyonu, Masako, Mendelsohn, Shaker ve supraglottik yutma gibi spesifik manevralar ile transkutanöz nöromuskuler elektriksel stimülasyon uygulamalarından faydalanılır. Besin miktarı ve kıvamının ayarlanması ile ekspiratuar kasları güçlendirici solunum rehabilitasyonu da hayati öneme sahiptir. Son dönemde ise transkraniyal manyetik stimülasyon ve özellikle Covid-19 pandemisinin etkisiyle uzaktan görsel eğitim ve terapi uygulamaları güncel gelişmeler olarak dikkat çekmektedir. Ülkemizde artan yaşlı nüfusla birlikte, donanımlı sağlık çalışanına ve YBR hizmetlerinin yaygınlaştırılmasına olan ihtiyaç her geçen gün artmaktadır.

Swallowing rehabilitation is a multidisciplinary approach aimed at overcoming difficulties in all phases of swallowing and eliminating the risk of aspiration. A well-equipped rehabilitation unit requires a coordinated team consisting of a physiatrist or otolaryngologist, a speech and language therapist, a nurse, and a dietitian. While neurological conditions, neuromuscular disorders, and head-neck malignancies constitute the primary etiologies in adults, systemic autoimmune and congenital diseases are prominent in younger populations. Treatment interventions include exercises for mouth opening and drooling, tongue strengthening, sensory tactile stimulation, and cervical postural maneuvers. Specialized swallowing maneuvers such as effortful swallow, Masako, Mendelsohn, Shaker, supraglottic swallow, and head-forward swallow are utilized alongside transcutaneous neuromuscular electrical stimulation. Adjusting food volume and consistency, combined with respiratory rehabilitation to strengthen expiratory muscles, is crucial to prevent life-threatening aspiration. Recent advancements include transcranial magnetic stimulation and telehealth applications, which gained significance during the Covid-19 pandemic. Given the growing elderly population, there is an increasing demand for comprehensive training of specialized healthcare professionals and wider accessibility of swallowing rehabilitation services.
