Özet

Akut abdomen, çoğunlukla ani başlayan karın ağrısı ile karakterize, hızlı tanı ve sıklıkla acil cerrahi müdahale gerektiren kritik bir klinik tablodur. Genel cerrahi acillerinin yaklaşık yarısını oluşturan bu durum, özellikle yaşlılarda, ek hastalığı olanlarda veya perfore kolon kanseri gibi vakalarda yüksek morbidite ve mortalite oranlarına sahiptir. Etiyolojisinde apandisit ve kolesistit gibi genel cerrahi nedenlerinin yanı sıra jinekolojik ve medikal patolojiler de yer alır. Tanı sürecinde, gelişmiş laboratuvar ve radyolojik tetkiklere rağmen detaylı anamnez ve sistematik fizik muayene halen altın standart olma özelliğini korumaktadır. Ağrının embriyolojik kökene göre değişen lokalizasyonu, karakteri (viseral, parietal veya yansıyan) ve hastanın tıbbi geçmişi ile kullandığı ilaçlar ayırıcı tanıyı daraltmada kritik öneme sahiptir. Fizik muayenede ise inspeksiyon, oskültasyon, perküsyon ve palpasyon sırasıyla uygulanmalıdır. İstemsiz defans, rebound ve Murphy bulgusu gibi spesifik manevralar peritoneal enflamasyonun ve kaynaklanan organın belirlenmesini sağlar. Erken ve doğru teşhis, hayati komplikasyonların önlenmesinde ve hasta sürvisinin artırılmasında temel faktördür.

Acute abdomen is a critical clinical condition characterized primarily by sudden-onset abdominal pain that requires prompt diagnosis and often urgent surgical intervention. Accounting for nearly half of all general surgery emergencies, it carries significant morbidity and mortality rates, particularly among the elderly, patients with comorbidities, or cases involving perforated colon cancers. Its etiology encompasses general surgical causes such as appendicitis and cholecystitis, as well as gynecological and medical conditions. In the diagnostic process, despite advanced laboratory and radiological investigations, a detailed medical history and a systematic physical examination still maintain their gold-standard status. The localization of pain according to embryological origin, its character (visceral, parietal, or referred), the patient's medical history, and current medications are critical for narrowing down the differential diagnosis. The physical examination must follow a strict sequence of inspection, auscultation, percussion, and palpation. Specific maneuvers and signs, including involuntary defense, rebound tenderness, and Murphy's sign, enable the detection of peritoneal inflammation and the identification of the affected organ. Early and accurate diagnosis remains the cornerstone for preventing life-threatening complications and improving patient outcomes.
