Özet
Ostomi, genel cerrahi pratiğinde elektif veya acil durumlarda sıkça uygulanan, ancak %70'e varan oranlarda yüksek komplikasyon riski barındıran önemli bir cerrahi işlemdir. Bağırsak malignensileri, travmalar ve anastomoz kaçakları gibi birçok durum ostomi endikasyonu oluşturmaktadır. İşlevlerine göre besleme, drenaj veya saptırıcı olarak sınıflandırılan ostomiler; parastomal herni, stoma prolapsusu, iskemi/nekroz, stoma retraksiyonu, yüksek drenaj ve kanama/hematom gibi aciliyet oluşturabilecek çeşitli komplikasyonlara yol açabilir. Yaş, obezite, diyabet ve acil cerrahi gibi faktörler bu riskleri artırırken, ameliyat öncesi stoma yerinin doğru işaretlenmesi ve cerrahların komplikasyon yönetimine hakim olması hayati önem taşır. Erken dönem sorunları genellikle konservatif yöntemlerle yönetilirken, geç dönem komplikasyonları ile iskemi veya tam obstrüksiyon gibi ciddi tablolar acil cerrahi müdahale ve revizyon gerektirmektedir.
Ostomy is a frequently performed surgical procedure in general surgery practice under both elective and emergency conditions, yet it carries a high complication rate of up to 70%. Various conditions, including intestinal malignancies, traumas, and anastomotic leaks, constitute indications for an ostomy. Classified as feeding, diversion, or drainage based on their function, ostomies can lead to several urgent complications such as parastomal hernia, stoma prolapse, ischemia/necrosis, stoma retraction, high output, and bleeding/hematoma. Factors like advanced age, obesity, diabetes, and emergency surgery increase these risks, whereas preoperative stoma site marking and surgeons' proficiency in complication management are highly critical. While early-term complications are generally managed with conservative treatments, late-term complications and severe conditions like ischemia or complete obstruction require urgent surgical intervention and revision.
