Motivasyonel Görüşme ve Hemşirelik
Özet
Motivasyonel Görüşme (MG), bireylerin davranış değişikliği konusundaki kararsızlıklarını (ambivalans) keşfetmelerine ve çözmelerine yardımcı olmayı amaçlayan, yönlendirici ve hasta odaklı terapötik bir danışmanlık yaklaşımıdır. İlk olarak klinik psikolog William Miller tarafından bağımlılık tedavisinde ortaya atılan bu teknik, daha sonra Stephen Rollnick ile geliştirilmiştir. MG'nin temel ilkeleri empati gösterilmesi, çelişkilerin ortaya çıkarılması, dirençle çalışma ve öz yeterliliğin desteklenmesinden oluşur. Sürecin etkinliğini artırmak için soru sorma, onaylama, yansıtmalı dinleme ve özetleme (SOYÖ) stratejileri ile "değişimi konuşma" yöntemleri kullanılır. Hemşirelik uygulamalarında MG, Transteorik Model rehberliğinde bireyin değişim aşamasını belirleyerek yaşam tarzı değişikliklerini kolaylaştırmada kritik bir role sahiptir. Sağlığı koruma, kronik hastalık yönetimi, sigara ve madde bağımlılığını önleme gibi koruyucu ve tedavi edici müdahalelerde sıkça kullanılır. Hemşirelik süreciyle tam bir uyum gösteren bu teknik; tanılama, planlama, uygulama ve değerlendirme aşamalarında hastayı güçlendirerek tedaviye uyumu ve yaşam kalitesini artırır. Sonuç olarak hemşirelerin MG tekniğini klinik iletişim becerisi olarak kullanması, toplumun sağlığını geliştirme davranışlarına çok yönlü ve olumlu katkılar sağlamaktadır.
Motivational Interviewing (MI) is a directive and patient-centered therapeutic counseling approach designed to help individuals explore and resolve their ambivalence regarding behavior change. Initially introduced by clinical psychologist William Miller for addiction treatment, this technique was later advanced into a theory alongside Stephen Rollnick. The core principles of MI encompass expressing empathy, developing discrepancy, rolling with resistance, and supporting self-efficacy. To maximize the efficiency of the process, four primary interaction strategies—questioning, affirming, reflective listening, and summarizing (OARS)—along with "change talk" methods are utilized. In nursing practices, guided by the Transtheoretical Model, MI plays a critical role in facilitating lifestyle modifications by identifying the patient's specific stage of change. It is widely applied in preventive, therapeutic, and rehabilitative interventions, such as health promotion, chronic disease management, and smoking or substance cessation. Aligning seamlessly with the nursing process, this technique empowers patients through the assessment, planning, implementation, and evaluation phases, thereby increasing treatment adherence and quality of life. Ultimately, integrating MI as a clinical communication skill enables nurses to contribute comprehensively and positively to public health development behaviors.
