Postoperatif Bakım ve Öneriler
Özet
Hemoroidektomi sonrası ağrı yönetimi ve rutin postoperatif bakım, hastanın konforu ve iyileşme sürecinin hızlandırılması açısından kritik bir öneme sahiptir. Ameliyat sonrası temel amaç, narkotik ilaçların kabızlık ve bağımlılık gibi yan etkileri nedeniyle kullanımlarını sınırlandırarak multimodal yöntemlerle ağrıyı azaltmaktır. Bu doğrultuda, pudental sinir bloğu uygulamaları ve lipozomal bubivakain kullanımı opioid gereksinimini belirgin şekilde azaltmaktadır. İlk basamak oral analjezide ise NSAİ ilaçlar ve parasetamol kombinasyonu tercih edilmektedir. Topikal uygulamalardan baklofen, gliseril trinitrat ve lokal anestezik kombinasyonlarının ağrıyı hafiflettiği bilinmektedir. Rutin bakım sürecinde katı gaita oluşumunu ve dolayısıyla dışkılama ağrısını engellemek amacıyla, operasyondan günler önce lif ve laksatif kullanımına başlanması önerilmektedir. Ayrıca, ılık suya oturma banyoları ile oral flavonoid fraksiyonu gibi venoaktif ajanların kullanımı da postoperatif ağrıyı, kanamayı ve hastanede kalış süresini azaltmada etkin rol oynamaktadır.
Postoperative pain management and routine care after hemorrhoidectomy are critical for enhancing patient comfort and accelerating recovery. The primary objective is to minimize the use of narcotic medications due to their side effects, such as constipation and addiction, by leveraging multimodal pain management strategies. In this context, pudendal nerve blocks and the utilization of liposomal bupivacaine significantly decrease the requirement for postoperative opioids. For first-line oral analgesia, a combination of NSAIDs and paracetamol is highly recommended. Additionally, topical applications including baclofen, glyceryl trinitrate, and local anesthetic combinations have been shown to alleviate pain. To avoid hard stools and subsequent painful defecation during routine postoperative care, initiating fiber and laxative intake a few days before surgery is established as a level 1 recommendation. Furthermore, warm sitz baths and oral venoactive agents, such as micronized flavonoid fractions, play an effective role in reducing postoperative pain, bleeding, and the duration of hospital stay.
