Özet

Lazer hemoroidoplasti (LHP), özellikle postoperatif ağrıyı azaltmak ve hastaların yaşam standardını yükseltmek amacıyla geliştirilmiş, eksizyon içermeyen, minimal invaziv bir cerrahi yöntemdir. Geleneksel eksizyonel teknikler nüks açısından başarılı olsa da yüksek ağrı düzeyleriyle ilişkilendirilirken; stapler yöntemleri ise farklı komplikasyon riskleri taşımaktadır. LHP yönteminde, anal vergeden yapılan küçük bir insizyondan ilerletilen 1470 nm diod lazer probu ile submukozal alana ve hemoroid pakesi içerisine noktasal atışlar uygulanarak pakenin sönmesi ve dejenere olması sağlanır. Literatürdeki çalışmalar, LHP'nin özellikle Evre 2 ve Evre 3 hemoroid hastalarında kısa operasyon süresi, düşük kanama oranları ve belirgin şekilde az postoperatif ağrı ile öne çıktığını, uzun dönemde ise yüksek hasta memnuniyeti sağladığını göstermektedir. Ancak, Evre 4 gibi ileri evre hastalıklarda etkinliğinin sınırlı olabileceği belirtilmektedir. Kanama, enfeksiyon ve ödem gibi komplikasyon oranları oldukça düşük olan LHP, güvenli bir günübirlik tedavi alternatifi sunmaktadır. Buna karşın, yöntemin henüz güncel tedavi kılavuzlarında yer almadığı ve klinik başarısının tam olarak netleşmesi için uzun dönem takip sürelerine sahip, geniş kapsamlı randomize kontrollü çalışmalara ihtiyaç duyulduğu vurgulanmaktadır.

Laser hemorrhoidoplasty (LHP) is a non-excisional, minimally invasive, and day-case surgical procedure developed to minimize postoperative pain and improve patients' quality of life. Although conventional resectional techniques achieve low recurrence rates, they are associated with significant postoperative pain, whereas stapled hemorrhoidopexy carries risks of distinct complications. The LHP technique involves creating a small incision near the anal verge, through which a 1470 nm diode laser probe is advanced beneath the mucosa to deliver precise energy pulses, successfully shrinking and degenerating the hemorrhoidal cushion. Existing literature demonstrates that LHP stands out for its short operative time, low bleeding rates, and remarkably low postoperative pain scores, particularly in patients with Grade 2 and Grade 3 hemorrhoids, yielding high long-term patient satisfaction. However, its efficacy may be limited in advanced cases such as Grade 4 disease. Complications like bleeding, infection, and edema remain low, making LHP a safe alternative. Nonetheless, it is noted that this method is not yet included in current medical guidelines, and further randomized controlled trials with extended follow-up periods are required to establish its definitive role.
