Özet
Akut hemoroidal kriz, genel cerrahi polikliniklerinde sıkça karşılaşılan, hastanın günlük aktivitelerini ciddi ölçüde kısıtlayan, şiddetli ağrı ve ödemle seyreden ıstırap verici bir klinik tablodur. Hem iç hem de dış hemoroidal hastalıklarda gelişebilen bu durum; eksternal pakelerin akut trombozu veya internal hemoroidlerin strangülasyonu, nekrozu ve ülserasyonu sonucu ortaya çıkar. Eksternal tromboz genellikle gençlerde, ağır yük kaldırma gibi ani içi basınç artışları sonrası tek bir pake olarak görülürken; internal tromboz çoklu pakeler şeklinde yapı gösterir. Tedavi yaklaşımında cerrahi ve non-operatif (konservatif) olmak üzere iki temel strateji mevcuttur. Akut eksternal trombozda ilk 4 gün içinde başvuran ve şiddetli ağrısı olan hastalarda acil hemoroidektomi tercih edilirken, 4 günden sonra başvuranlarda non-operatif yöntemler uygulanır. İnternal hemoroid krizlerinde ise sepsis ve anal darlık gibi komplikasyon riskleri nedeniyle öncelikle yatak istirahatı, diyet düzenlemeleri, sıcak oturma banyoları ve ilaç tedavisini içeren non-operatif yöntemler seçilir. Geçmişte uygulanan trombektomi yöntemi, yüksek nüks oranı nedeniyle günümüzde terk edilmiştir. Çıkarılan tüm cerrahi spesmenlerin, nadir görülen anal kanser veya enfeksiyon patolojilerini dışlamak adına istisnasız patolojik incelemeye gönderilmesi zorunludur.
Acute hemorrhoidal crisis is a highly painful condition frequently encountered in general surgery clinics, severely limiting patients' daily activities due to intense pain and edema. This complication can arise in both internal and external hemorrhoidal diseases, presenting as acute thrombosis of external hemorrhoids or strangulation, necrosis, and ulceration of prolapsed internal hemorrhoids. While external thrombosis typically occurs as a single pile in young individuals following sudden intra-abdominal pressure increases, internal thrombosis usually manifests as multiple piles. Treatment approaches involve both surgical and non-operative strategies. In acute external thrombosis, emergency hemorrhoidectomy is recommended for patients presenting with severe pain within the first 4 days, whereas non-operative management is preferred for those presenting later. For internal hemorrhoidal crises, non-operative measures—including bed rest, warm sitz baths, dietary modifications, and medical therapy—are primarily selected to avoid severe complications such as sepsis and anal stricture. Thrombectomy has been abandoned in modern surgical practice due to high recurrence rates. Crucially, all surgical specimens must undergo routine pathological evaluation to rule out rare conditions like anal cancer or underlying chronic infections.
