Özet

Cerrahide başarılı bir sütürasyon için dokuda az travma oluşturulması, dokuların tam karşılıklı gelmesi ve uygun materyal seçimi temel prensiplerdir. Cilt altı sütürlerde, gerginliği azaltan basit teknik ile daha hızlı ve dayanıklı olan ancak ayrışma riski taşıyan devamlı teknik alternatiflerdir; her ikisinde de kısa sürede emilebilen multiflaman sütürler tercih edilir. Cilt sütürlerinde en yaygın yöntem, gerim kuvveti yüksek basit cilt sütürüdür. Daha az iz bırakan devamlı cilt sütürü ile estetik amaçlı kullanılan devamlı subkutan sütür de diğer seçeneklerdir ve bu tekniklerde emilemeyen monoflaman materyaller kullanılır. Özel sütürler arasında, enfeksiyon riskinde cilt altı dikiş yerine kullanılan Allgöver; yüksek gerilimli yaralarda dokuyu destekleyen ancak iz bırakma riski olan Mattress (yatay/dikey); ve düzensiz yaralarda doku nekrozunu önleyen köşe sütürü yer alır. Sonuç olarak, cerrahi müdahalelerde doku özelliklerine ve enfeksiyon durumuna göre en doğru teknik seçilmeli, gerginliksiz ve minimum travmalı bir kapatma sağlanmalıdır.

In surgery, the fundamental principles for successful suturing include causing minimal tissue trauma, ensuring precise tissue approximation, and selecting appropriate suture materials. Subcutaneous sutures offer alternatives such as the simple technique, which reduces tension, and the continuous technique, which is faster and more durable but carries a risk of wound dehiscence; both utilize short-term absorbable multifilament sutures. Among skin sutures, the simple skin suture is the most common method due to its high tensile strength, while the continuous skin suture leaves fewer scars, and the continuous subcuticular suture is preferred for aesthetic concerns, both requiring non-absorbable monofilament materials. Special sutures include the Allgöver suture, used instead of subcutaneous stitches in high infection risks; the Mattress suture (horizontal/vertical), which supports tissues in high-tension wounds but risks scarring; and the corner suture, which prevents tissue necrosis in irregular wounds. Consequently, the most appropriate suturing technique must be selected based on the incision site, size, and infection status to ensure a tension-free closure with minimal trauma.
