Endoskopik Cerrahinin Tarihçesi ve Geleceği
Özet
Endoskopik cerrahi, Latince kökenli kelimelerin birleşimiyle "içi boş kaviteleri değerlendiren enstrüman" olarak tanımlanmakta ve tarihsel gelişimini insanların kapalı kaviteleri keşfetme merakına borçludur. İlk adımlar Arap hekim Albukasim’in ışığı yansıtarak serviksi incelemesiyle başlamış, 1805'te Bozzini insan üretrasını gözlemlemiştir. 1901'de Kelling köpeklerde ilk laparoskopiyi yapmış, 1910'da Jacobaeus ise insanlarda ilk klinik uygulamaları gerçekleştirmiştir. 1960’larda jinekolog Palmer küçük cerrahi girişimleri başlatmış, Kurt Semm ise otomatik insuflasyon ve Roeder Loop gibi cihazlar geliştirerek modern cerrahinin temelini atmıştır. Semm'in jinekoloji dışındaki ilk laparoskopik apendektomiyi yapması genel cerrahlarca yoğun eleştirilse de, multidisipliner kullanımın önünü açmıştır. 1980'lerde Mouret, Dubois ve Perissat'nın çalışmalarıyla laparoskopik kolesistektomi Avrupa'da yayılmış, CAES ve SAGES gibi dernekler kurularak uluslararası iş birlikleri ile "Surgical Endoscopy" dergisi çıkarılmıştır. 1983'te Wickham "Minimal İnvazif Cerrahi" terimini ortaya atmış, bilgisayar çiplerinin ve CCP mekanizmalı kameraların entegrasyonuyla cerrahların konforu ve operasyonel kabiliyeti devrimsel şekilde artmıştır. Günümüzde ise makine öğrenimi algoritmaları, robotik kollar ve simülasyon teknolojileri ile cerrahide esansiyel bir dijital devrimin arefesinde bulunulmaktadır.
Endoscopic surgery is defined as an "instrument used to evaluate hollow cavities," merging Latin roots, and owes its historical evolution to human curiosity about exploring closed cavities. The earliest steps began with the Arabian physician Albukasim examining the cervix using reflected light, followed by Bozzini observing the human urethra in 1805. In 1901, Kelling performed the first laparoscopy on dogs, and in 1910, Jacobaeus conducted the first clinical applications in humans. In the 1960s, gynecologist Palmer initiated minor surgical interventions, while Kurt Semm laid the foundation for modern surgery by developing devices like automatic insufflation and the Roeder Loop. Although Semm's execution of the first laparoscopic appendectomy outside of gynecology faced harsh criticism from general surgeons, it pioneered multidisciplinary usage. In the 1980s, through the work of Mouret, Dubois, and Perissat, laparoscopic cholecystectomy expanded across Europe, leading to the establishment of associations like CAES and SAGES, and the publication of the journal "Surgical Endoscopy" through international collaborations. In 1983, Wickham coined the term "Minimally Invasive Surgery," and the integration of computer chips and cameras with CCP mechanisms revolutionarily enhanced surgeons' comfort and operational capabilities. Today, with machine learning algorithms, robotic arms, and simulation technologies, surgery stands on the brink of an essential digital revolution.
