Özet

Yara iyileşmesi; hemostaz, enflamasyon, proliferasyon ve yeniden yapılanma (HEPR) olmak üzere birbiri ardınca devam eden dört dinamik aşamadan oluşan biyolojik bir süreçtir. Bu sürecin başarılı bir şekilde tamamlanması için her evrenin uygun sıra, zaman ve yoğunlukta gerçekleşmesi gerekir. Yara iyileşmesini etkileyen faktörler lokal ve sistemik olarak ikiye ayrılır. Lokal faktörlerden oksijenlenme hücre metabolizması ve ATP üretimi için kritik olup, yetersizliği durumunda gelişen kronik hipoksi iyileşmeyi geciktirir. Enfeksiyonlar ise proinflamatuar sitokinlerin ortamda uzun süre kalmasına ve inflamatuar fazın uzayarak yaranın kronikleşmesine yol açar. Sistemik faktörlerden ileri yaş, uzamış inflamasyon ve azalan hücre fonksiyonu nedeniyle iyileşme sürecini yavaşlatır. Psikolojik stres, glukokortikoid seviyesini artırıp proinflamatuar sitokinleri azaltarak immün yanıtı ve iyileşmeyi olumsuz etkiler. Diyabette kronik hipoksi, yüksek metaloproteaz seviyeleri ve nöropati iyileşmeyi bozar. Glukokortikoid steroidler, NSAİİ'ler ve kemoterapötik ilaçlar hücresel yanıtları baskılayarak süreci geciktirir. Obezite subkutan yağ dokusundaki hipoperfüzyon ve iskemiden dolayı, alkol tüketimi anjiyogenezi baskılayarak, sigara içmek ise nikotin ve karbonmonoksit kaynaklı doku hipoksisi nedeniyle iyileşmeyi baltalar. Son olarak protein, karbonhidrat, amino asitler (arginin, glutamin), C, A ve E vitaminleri ile çinko, bakır ve demir gibi mineralleri içeren zengin bir beslenme desteği optimum doku onarımı için şarttır.

Wound healing is a dynamic biological process consisting of four sequential phases: hemostasis, inflammation, proliferation, and remodeling (HIPR), which must occur in the proper order, timeframe, and intensity for successful recovery. The factors influencing this process are categorized as local and systemic. Among local factors, oxygenation is crucial for cellular metabolism and ATP production, whereas prolonged or chronic hypoxia delays healing. Infections cause pro-inflammatory cytokines to persist, prolonging the inflammatory phase and leading to non-healing chronic wounds. Regarding systemic factors, advanced age slows the process due to prolonged inflammation and altered macrophage functions. Psychological stress impairs immune response and delays healing by increasing glucocorticoid levels and decreasing pro-inflammatory cytokines. In diabetes, chronic hypoxia, high metalloprotease levels, and neuropathy impede recovery. Medications such as glucocorticoid steroids, NSAIDs, and chemotherapeutic drugs delay healing by suppressing cellular responses. Additionally, obesity impairs healing through hypoperfusion and ischemia in adipose tissue, alcohol consumption disrupts proliferation by suppressing angiogenesis, and smoking undermines repair via nicotine and carbon monoxide-induced tissue hypoxia. Finally, proper nutritional support containing proteins, carbohydrates, amino acids (arginine, glutamin), vitamins C, A, and E, as well as minerals like zinc, copper, and iron, is essential for optimal tissue repair.
