İnfeksiyon: Kültür Sonucuna Göre Antibiyotik Seçimi
Özet
Kronik yaralarda enfeksiyon bulunmadıkça antibiyotik kullanılmamalı, aksi takdirde bakteriyel direnç, yan etki ve maliyet artışı gelişebilmektedir. Enfeksiyon varlığında debridman sonrası derin dokudan hem aerobik hem de anaerobik kültür alınarak ampirik tedaviye başlanmalı, ardından ideal olarak kültür antibiyogram sonucuna göre en dar spektrumlu ilaç seçilmelidir. Diyabetik ayak enfeksiyonları genellikle polimikrobiyal olup yüzeyel vakalarda gram pozitif koklar etkili iken, derin ve nekrotik yaralarda Enterobacteriaceae, Pseudomonas aeruginosa ve anaeroplar sürece dahil olmaktadır. Bası yaralarında fekal kontaminasyon ve polimikrobiyal kolonizasyon iyileşmeyi geciktirirken; venöz staz ülserlerinde rutin sistemik antibiyotik kullanımı yerine debridman önerilmekte, antibiyoterapi yalnızca akut sellülit gibi klinik enfeksiyon durumlarında uygulanmaktadır. Son yıllarda genişlemiş spektrumlu beta-laktamaz (GSBL) üreten dirençli gram negatif basiller, karbapenem dirençli suşlar ve metisiline dirençli S. aureus (MRSA) enfeksiyonları küresel bir risk oluşturarak tedavi seçeneklerini kısıtlamaktadır. Türkiye'de yapılan çok merkezli çalışmalar da yara kültürlerinde yüksek oranda gram negatif etkenler ve antibiyotik direnci saptandığını ortaya koymaktadır. Hafif enfeksiyonlarda etkenin türüne göre oral amoksisilin/klavulonat, klindamisin veya siprofloksasin gibi seçenekler kullanılırken; orta ve ağır enfeksiyonlarda parenteral penisilin G, sefazolin, vankomisin, meropenem veya daptomisin gibi spesifik tedavi protokolleri tercih edilmektedir. Sonuç olarak, etkin bir kronik yara yönetimi için direnç durumlarının yakından takibi ve akılcı antibiyotik kullanımı esastır.
Antibiotics should not be used in chronic wounds unless infection is present, otherwise bacterial resistance, side effects, and increased costs may develop. In the presence of infection, empirical treatment should be initiated after obtaining both aerobic and anaerobic cultures from deep tissue post-debridement, and ideally, the narrowest spectrum drug should be selected based on culture antibiogram results. Diabetic foot infections are generally polymicrobial; while gram-positive cocci are active in superficial cases, Enterobacteriaceae, Pseudomonas aeruginosa, and anaerobes are involved in deep and necrotic wounds. While fecal contamination and polymicrobial colonization delay healing in pressure ulcers, debridement is recommended for venous stasis ulcers instead of routine systemic antibiotics, which are reserved only for clinical infections like acute cellulitis. In recent years, resistant gram-negative bacilli producing extended-spectrum beta-lactamase (ESBL), carbapenem-resistant strains, and methicillin-resistant S. aureus (MRSA) infections pose a global risk, limiting treatment choices. Multi-center studies in Turkey also reveal high rates of gram-negative agents and antibiotic resistance in wound cultures. For mild infections, oral options like amoxicillin/clavulanate, clindamycin, or ciprofloxacin are used depending on the pathogen type, whereas specific parenteral treatment protocols such as penicillin G, cefazolin, vancomycin, meropenem, or daptomycin are preferred for moderate to severe infections. Consequently, close monitoring of resistance patterns and rational antibiotic use are essential for effective chronic wound management.
