Kronik Yara Hastası Bakımında Psikolojik Destek: Kronik Yarası Olan Bireylerin Psikolojik Sorunlarına Yaklaşım ve Tedavi Yönetimi

Özet

Bu metin, kronik yarası olan bireylerin yaşadığı psikolojik sorunları, tedavi yönetimini ve bakım verenlerin yükünü ele almaktadır. Kronik yara ile yaşamak; ağrı, işlevsellik kaybı, sosyal izolasyon ve yalnızlık gibi olumsuzluklara yol açarak bireylerin yaşam kalitesini ve iyilik halini belirgin şekilde düşürmektedir. Hastaların en az %30'unda depresyon ve anksiyete bozukluğu görülmekte olup, bu psikolojik sorunlar yara iyileşmesini geciktirmekte ve yeni yaraların oluşumunu tetikleyebilmektedir. Ruhsal bozukluk tanısı için DSM-5 kriterlerine göre işlevsellik kaybı aranmaktadır. Klinisyenlerin hastaları bütüncül bir yaklaşımla, hem sözel hem de gözleme dayalı olarak değerlendirmesi, gerektiğinde ruh sağlığı profesyonellerine yönlendirmesi önemlidir. Tedavide SSRI, SNRI ve trisiklik antidepresanlar gibi medikal çözümlerin yanı sıra sosyal destek sağlayan grup çalışmaları da iyileşme sürecine katkı sunar. Şizofreni ve zihinsel yetersizlik gibi özellikli hasta gruplarında öz bakım eksikliği ve tütün kullanımı gibi riskler nedeniyle süreç daha karmaşıktır; bu nedenle net ve tekrarlayan iletişim ile bakım veren desteği şarttır. Ayrıca, kronik stres altında olan aile içi bakım verenlerde tükenmişlik ve depresyon sıklıkla gelişmekte, bu durum bakım kalitesini düşürmektedir. Sonuç olarak, yara tedavisinde hastanın konforunu artırmak ve eşlik eden ruhsal sorunları iyileştirmek için multidisipliner ve biyopsikososyal bütünsel bir yaklaşım zorunludur.

This text addresses the psychological problems experienced by individuals with chronic wounds, treatment management, and the burden on caregivers. Living with a chronic wound significantly reduces individuals' quality of life and well-being by leading to negativity such as pain, loss of functionality, social isolation, and loneliness. Depression and anxiety disorders are observed in at least 30% of patients, and these psychological problems delay wound healing and can trigger the formation of new wounds. According to DSM-5 criteria, a loss of functionality is required for the diagnosis of a mental disorder. It is important for clinicians to evaluate patients with a holistic approach, both verbally and observationally, and to refer them to mental health professionals when necessary. In addition to medical solutions such as SSRIs, SNRIs, and tricyclic antidepressants, group activities providing social support also contribute to the healing process. In special patient groups such as schizophrenia and intellectual disability, the process is more complex due to risks like lack of self-care and tobacco use; therefore, clear and repeated communication along with caregiver support is essential. Furthermore, burnout and depression frequently develop in family caregivers under chronic stress, which reduces the quality of care. Consequently, a multidisciplinary and biopsychosocial holistic approach is mandatory in wound treatment to increase patient comfort and heal accompanying mental problems.
