Soliter Pulmoner Nodül
Özet
Soliter pulmoner nodül (SPN), akciğerde 3 cm’den küçük, atelektazi veya lenfadenopati gibi durumların eşlik etmediği lokal opasitelerdir. Tarama programlarının yaygınlaşmasıyla tespiti artan SPN’lerin %40-60 oranında malign olma riski bulunmaktadır. En etkin radyolojik değerlendirme yöntemi toraks bilgisayarlı tomografisidir (BT). Nodülün büyüme hızı, çapı, kenar özellikleri ve yoğunluğu (solid, yarı solid veya buzlu cam) malignite olasılığını belirlemede kritik parametrelerdir. Türkiye'nin demografik yapısında en sık benign nedenler tüberküloz sekeli ve hamartom iken, malign nedenlerin başında primer akciğer kanserleri gelir. Tanı ve risk hesaplamasında yerel verilerle geliştirilen Yedikule-SPN skorlama modeli, Brock modeline göre ülkemiz popülasyonunda daha yüksek duyarlılık ve özgüllük göstermiştir. Yönetim sürecinde rehberler doğrultusunda takip, biyopsi veya cerrahi rezeksiyon stratejileri uygulanır.
A solitary pulmonary nodule (SPN) is defined as a local opacity in the lung measuring less than 3 cm, without associated atelectasis or lymphadenopathy. With the widespread use of screening programs, SPN detection has increased, carrying a 40-60% risk of malignancy. Chest computed tomography (CT) remains the gold standard for radiological evaluation. Key parameters for determining the likelihood of malignancy include growth rate, size, border characteristics, and density (solid, semi-solid, or ground-glass). In Turkey's demographic structure, tuberculosis sequelae and hamartoma are the most common benign etiologies, whereas primary lung cancers represent the leading malignant causes. For diagnosis and risk assessment, the locally developed Yedikule-SPN scoring model has demonstrated superior sensitivity and specificity in the Turkish population compared to the Brock model. Management strategies involve follow-up, biopsy, or surgical resection guided by established international guidelines.
