Özet

Trakeostomi, günümüzde genellikle uzamış mekanik ventilasyon ihtiyacı olan yoğun bakım hastalarında uygulanan ve endotrakeal entübasyona göre hasta konforu, sedasyon ihtiyacında azalma ile pulmoner temizlik gibi avantajlar sağlayan cerrahi bir prosedürdür. Tarihsel kökeni M.Ö. 3600’lü yıllara dayanan bu işlem, cerrahi ve perkütan olmak üzere iki ana teknikle gerçekleştirilmektedir. Cerrahi yöntem genellikle ameliyathane şartlarında genel anestezi altında uygulanırken, perkütan trakeostomi yöntemi yatak başında, daha düşük maliyetle ve fiberoptik bronkoskop veya ultrason kılavuzluğunda konforlu bir şekilde yapılabilmektedir. İşlemin uygulanması için kesin bir zaman kısıtlaması olmamakla birlikte, genellikle 7-10 gün boyunca entübe kalan ve yakın zamanda ekstübe edilmesi beklenmeyen hastalar için planlama yapılması önerilmektedir. Trakeostomi uygulamalarında minimal semptomlardan hayatı tehdit eden durumlara kadar uzanabilen peroperatif, erken postoperatif ve geç dönem komplikasyonlar görülebilmektedir. Bunlar arasında kanama, tüpün yerinden çıkması, enfeksiyon gibi erken sorunların yanı sıra trakeal stenoz, trakeomalazi ve trakeaözofajial fistül gibi ciddi geç dönem komplikasyonları yer almaktadır. Bu riskleri en aza indirmek adına, kaf basıncının kontrolü, uygun kanül seçimi ve deneyimli personel tarafından düzenli yara bakımı gibi işlem sonrası takip prosedürlerine hassasiyetle uyulması hayati önem taşımaktadır.

Tracheostomy is a common surgical procedure frequently utilized in intensive care units, primarily for patients requiring prolonged mechanical ventilation, offering advantages such as enhanced patient comfort, reduced sedation needs, and facilitated pulmonary clearance compared to endotracheal intubation. With historical origins dating back to 3600 BC, the procedure is currently performed using two main techniques: open surgical and percutaneous tracheostomy. While the surgical method is traditionally performed under general anesthesia in an operating room, the percutaneous technique has gained significant popularity as a cost-effective bedside alternative, typically guided by fiberoptic bronchoscopy or ultrasound. Although there is no definitive consensus on timing, the procedure is generally planned for patients who have been intubated for 7-10 days and are not expected to be extubated in the near future. Tracheostomy involves various perioperative, early postoperative, and late complications ranging from mild symptoms to life-threatening conditions. Early risks include hemorrhage, accidental tube dislodgement, and stoma infection, whereas late complications encompass tracheal stenosis, tracheomalacia, and tracheoesophageal fistula. To mitigate these risks and ensure airway safety, strict adherence to postoperative care, including monitoring cuff pressure, selecting appropriate cannula sizes, and ensuring management by experienced staff, is highly crucial.
