Özet

Trakea cerrahisinde cerrahi yaklaşımı lezyonun yerleşimi belirlemekte olup, operasyon öncesinde fiberoptik/rijit bronkoskopi ve bilgisayarlı tomografi ile vokal kordların ve lezyonun tam yeri, yayılımı ve kalan trakea uzunluğu değerlendirilmelidir. Subglottik ve servikal lezyonlarda genellikle kollar insizyon yeterli olurken, yaşlı veya kısa boyunlu hastalarda parsiyel sternotomi eklenebilir. Midtrakeal lezyonlarda kollar kesiye ek olarak parsiyel/komplet sternotomi veya sağ anterolateral torakotomi gerekebilir; alt trakea ve karinal lezyonlarda ise en iyi yaklaşım sağ posterolateral torakotomi ile sağlanır. Anastomoz gerilimini azaltmak amacıyla baş fleksiyonu, anterior pretrakeal parmak diseksiyonu, supratiroid/suprahyoid laringeal serbestleştirme ve hiler serbestleştirme gibi mobilizasyon manevraları uygulanır. Diseksiyon esnasında segmental beslenmenin korunması, rekürren laringeal sinir hasarının önlenmesi ve benign vakalarda sinirin fibröz dokuda bırakılması kritiktir. Brakiosefalik arter fistülünü önlemek için koruyucu fibröz doku korunmalı veya araya strap kas pedikülü yerleştirilmelidir. Rezeksiyon sonrası distal hava yolu entübe edilerek ventilasyon sürdürülür. Anastomoz, lateral destek sütürleri yerleştirilip baş fleksiyona getirildikten sonra, posterior membranöz yüzde kontunyu/interrupted PDS, anterior kartilaj yüzde ise interrupted Vicryl sütürlerle lümen dışında düğümlenerek tamamlanır. Kaçak kontrolü sonrası cerrahi sahaya dren yerleştirilir, katlar kapatılır ve boyun ekstansiyonunu önlemek için çene-sternum arasına gardiyan sütür atılır. Subglottik stenozlarda ise krikoid anterior ark rezeksiyonu ve lümeni genişletmek için tirotomi gerekebilir.

In tracheal surgery, the surgical approach is dictated by the location of the lesion, requiring preoperative evaluation via fiberoptic/rigid bronchoscopy and computed tomography to determine the vocal cords, exact site, extension, and estimated residual tracheal length. A collar incision is generally sufficient for subglottic and cervical lesions, though partial sternotomy may be added in elderly or short-necked patients. Midtracheal lesions may require a collar incision combined with partial/complete sternotomy or right anterolateral thoracotomy, whereas thoracic lower tracheal and carinal lesions are best approached via right posterolateral thoracotomy. To reduce anastomotic tension, mobilization maneuvers such as head flexion, anterior pretracheal digital dissection, suprathyroid/suprahyoid laryngeal release, and hilar release are performed. During dissection, preserving segmental blood supply and avoiding recurrent laryngeal nerve injury are critical; in benign cases, leaving the nerve within fibrous tissue is recommended. To prevent brachiocephalic artery fistula, the protective fibrous tissue must be preserved or a strap muscle pedicle interposed. Following resection, ventilation is maintained via distal airway intubation. After placing lateral stay sutures and bringing the head into flexion, the anastomosis is completed using continuous/interrupted PDS for the posterior membranous wall and interrupted Vicryl for the anterior cartilage, keeping knots extraluminal. After checking for leaks, a drain is placed, layers are closed, and a guardian suture is applied to prevent neck extension. Subglottic stenosis may require cricoid resection and thyrotomy.
