Özet

Dünyada milyonlarca insanı etkileyen parazitik enfeksiyonlar, küresel ısınma, seyahatler ve artan immünsüpresif nüfus nedeniyle endemik bölgelerin dışına da yayılmaktadır. Klinik ve radyolojik bulguların nonspesifik olması tanıyı zorlaştırırken, seyahat öyküsü ve hijyen sorgulaması önem taşımaktadır. Bölgede en sık görülen paraziter hastalık, Echinococcus türlerinin neden olduğu kistik hidatittir. Enfekte köpeklerle temas veya kontamine gıdalarla bulaşan bu parazit, toraks içerisinde en sık akciğerin sağ alt lobunu tutar. Kistler öksürük, ateş ve nefes darlığına; rüptür durumunda ise kaya suyu ekspektorasyonu ile anafilaktik şok gibi komplikasyonlara yol açar. Tanıda sınırları belirgin sıvı dansiteli lezyonlar, nilüfer arazı gibi radyolojik görüntüler, eozinofili ve yüksek IgE seviyeleri değerlidir. Tedavide ana yaklaşım, Barrett veya Perez-Fontana gibi akciğer dokusunu koruyan cerrahi yöntemlerdir. Antihelmintik medikal tedaviler düşük kür oranına sahiptir ve rüptür riskini artırabilir. Günümüzde uygun hastalarda geleneksel torakotomi yerine tercih edilen Video-yardımlı torakoskopik cerrahi (VATS), çift lümenli tüp entübasyonu ile iki insizyondan uygulanır. VATS, açık cerrahiye kıyasla daha az postoperatif ağrı, daha kısa cerrahi ve drenaj süresi ile daha iyi estetik sonuçlar sunmaktadır.

Parasitic infections affecting millions worldwide are expanding beyond endemic areas due to global warming, international travel, and a growing immunosuppressed population. Diagnosis remains difficult due to nonspecific clinical and radiological findings, making travel and hygiene history essential. The most common pulmonary parasitic disease in the region is cystic hydatid disease caused by Echinococcus species. Transmitted through contact with infected dogs or ingestion of contaminated food, the larvae mostly target the right lower lobe of the lung. Cysts cause cough, fever, and dyspnea, and their rupture can lead to expectoration of cyst fluid, anaphylactic shock, or sepsis. Diagnosis utilizes well-defined fluid-density lesions or water-lily signs on radiology, alongside elevated IgE and eosinophilia. The primary treatment is lung-preserving surgery, such as the Barrett or Perez-Fontana techniques. Medical treatments show low cure rates and may increase spontaneous rupture risks. Replacing traditional thoracotomy, modern Video-Assisted Thoracoscopic Surgery (VATS) is performed via double-lumen intubation and two incisions. VATS provides superior advantages over open surgery, including less postoperative pain, shorter surgical and drainage times, and better cosmetic outcomes for eligible patients.
