Özet

Pulmoner aspergillozis, genellikle akciğer parankimindeki yüzeylerde ve tüberküloz, bronşektazi, akciğer absesi gibi önceden var olan nekroz kalıntılarının ve kavitelerin içinde kolonize olabilen opportunist bir mantar enfeksiyonudur. En sık izole edilen etken Aspergillus fumigatus olup; hastalık alerjik, invaziv ve aspergillom olmak üzere üç ana klinik tablo oluşturur. Akciğer kavitelerinde hifa, fibrin ve inflamatuar hücrelerin birleşerek oluşturduğu nekrotik kitleye aspergilloma veya "fungus ball" denir. Hastalarda en sık görülen ve tedaviyi yönlendiren semptom, fungus topunun vasküler yapıları erode etmesi veya salgıladığı toksinler nedeniyle bronşiyal arterlerden kaynaklanan hemoptizidir. Tanıda düz akciğer grafisinde görülen "Monod belirtisi" (air-crescent sign) ve tomografi bulguları tipiktir. Kesin tanı histopatolojik olarak aspergillusun gösterilmesiyle konur. Pulmoner aspergillomanın medikal tedavisi efektif olmadığından temel tedavi seçeneği cerrahidir. Hayatı tehdit eden hemoptizi başlıca cerrahi endikasyonudur. Ameliyat tolere edilebiliyorsa, ileride gelişebilecek komplikasyonları önlemek adına asemptomatik hastalarda da cerrahi gecikmeden önerilir. En çok tercih edilen yöntem lobektomidir; ancak solunum rezervi yetersiz hastalarda kavernostomi veya kavernomyoplasti de uygulanabilir. Cerrahide komplikasyon oranları altta yatan hastalığa bağlı olarak gelişebilir.

Pulmonary aspergillosis is an opportunistic fungal infection of the lung that colonizes surfaces within the lung parenchyma and necrotic cavities caused by conditions such as tuberculosis, bronchiectasis, or lung abscess. The most frequently isolated pathogen is Aspergillus fumigatus, presenting primarily in three clinical forms: allergic, invasive, and aspergilloma. An aspergilloma, or fungus ball, is a necrotic mass containing hyphae, fibrin, and inflammatory cells formed within a pre-existing lung cavity. Hemoptysis, arising mostly from bronchial arteries due to vascular erosion by the fungus ball or its released toxins and enzymes, is the most common symptom and key determinant for treatment. Diagnosis is typically indicated by the "air-crescent sign" (Monod's sign) on chest X-rays and thoracic computed tomography. Definitive diagnosis requires demonstrating the fungus in aspiration or resection specimens. Since medical treatment is considered ineffective, surgery remains the primary therapeutic option for pulmonary aspergilloma. Life-threatening hemoptysis is the chief surgical indication; however, early surgical intervention is recommended even for asymptomatic patients who can tolerate it to prevent future fatal complications. While lobectomy is the most preferred procedure, cavernostomy or cavernomyoplasty can be performed in patients with poor respiratory reserve.
