Ereksiyon Fonksiyon Bozukluğu (EFB) Tedavisinde Kullanılan Düşük Yoğunluklu Ekstrakorporeal Şok Dalga Terapisi (EŞDT) ile Yapılan Deneysel Çalışmalar ve Sonuçları
Özet
Ereksiyon fonksiyon bozukluğu (EFB), özellikle orta yaş ve üzeri erkeklerde yaşam kalitesini düşüren, vasküler, nöral ve hormonal faktörlere bağlı yaygın bir rahatsızlıktır. Mevcut tedaviler (PDE5i, intrakavernozal enjeksiyonlar, penil protez vb.) geçici çözümler sunmakta ve hastalığın altındaki temel patolojiyi düzeltememektedir. Bu bağlamda, Düşük Yoğunluklu Ekstrakorporeal Şok Dalga Terapisi (EŞDT / Li-ESWT), kalıcı ve kür sağlayan minimal invaziv bir alternatif olarak öne çıkmaktadır. Deneysel çalışmalar, EŞDT'nin erken dönemde nNOS ve eNOS aktivitesini artırarak nitrik oksit yapımını ve kan akışını uyardığını göstermektedir. Orta ve uzun dönemde ise vazoaktif endotelyal büyüme faktörlerini (VEGF) ve kök hücreleri aktive ederek yeni damar oluşumunu (neoanjiyogenez) tetikler. Ayrıca nNOS pozitif sinir liflerini onararak nöral rejenerasyon sağlar, BDNF uyarımı ile sinir hasarlarını iyileştirir ve radikal prostatektomi sonrası impotans için umut vadeder. Kavernozal dokudaki fibrotik değişimleri (skarlaşmayı) engelleyerek dokuyu yeniden yapılandırır. Son olarak, alfa-2 adrenerjik reseptör lehine değişim yaratarak sempatik tonusu azaltır ve performans anksiyetesine bağlı EFB'de de etkili olabileceğini gösterir. Sonuç olarak preklinik çalışmalar, EŞDT'nin yaşlanma, diyabet veya cerrahi hasar kaynaklı patolojileri hücresel düzeyde onardığını kanıtlamaktadır.
Erectile dysfunction (ED) is a common disorder linked to vascular, neural, and hormonal factors that significantly impairs the quality of life, particularly in middle-aged and older men, and while current treatments like PDE5 inhibitors, intracavernosal injections, or penile prostheses offer temporary solutions, they fail to correct the underlying pathology, which positions Low-Intensity Extracorporeal Shock Wave Therapy (Li-ESWT) as a promising, non-invasive, curative alternative; in fact, pre-clinical studies demonstrate that Li-ESWT stimulates nitric oxide production and blood flow in the short term by upregulating nNOS and eNOS activity, while in the medium-to-long term, it triggers neoangiogenesis by activating vasoactive endothelial growth factors (VEGF) and recruiting progenitor stem cells, induces neural regeneration and repair through BDNF stimulation—offering potential for post-prostatectomy impotence—and exerts antifibrotic effects that remodel cavernosal tissue to prevent aging-related scarring, furthermore, it modulates sympathetic tone by shifting the alpha-1/alpha-2 adrenergic receptor ratio in favor of alpha-2 receptors to alleviate performance anxiety, ultimately proving that shockwave therapy effectively repairs arterial, neural, and fibrotic tissue damage caused by aging, diabetes, or surgical trauma at a cellular level.
