Özet

Ereksiyon bozukluğu (EB), tatminkâr cinsel aktivite için yeterli penis sertliğinin sağlanamaması durumudur ve güncel tedaviler (PDE5İ, enjeksiyonlar, implantlar) genellikle semptomatik olup altta yatan patolojiyi düzeltmez. Bu durum, araştırmacıları rejeneratif ve doku iyileştirici özellikleriyle öne çıkan intrakavernozal trombositten zengin plazma (PRP) tedavisine yöneltmiştir. PRP, hastanın kendi kanından ayrıştırılan yüksek yoğunluktaki büyüme faktörlerini (VEGF, PDGF, FGF, IGF-1) barındırır. Bu faktörler; yeni damar oluşumunu, düz kas hücrelerinin proliferasyonunu ve nöral rejenerasyonu tetikleyerek etki gösterir. Hayvan modellerinde yapılan preklinik çalışmalar, PRP enjeksiyonunun kavernozal sinir hasarı sonrası akson koruması sağladığını ve erektil fonksiyonları belirgin şekilde iyileştirdiğini kanıtlamıştır. Klinik araştırmalar ve meta-analizler ise yöntemin güvenilir, fizibil ve düşük komplikasyon riskine sahip olduğunu, hastaların IIEF-5 skorlarında ve tedavi memnuniyetlerinde anlamlı artışlar sağladığını ortaya koymaktadır. Özellikle sigara içmeyenlerde ve başlangıç IIEF skorları yüksek olan hastalarda tedavi başarısı çok daha belirgindir. İlk çift-kör plasebo kontrollü çalışma da PRP'nin kontrol grubuna kıyasla cinsel işlev üzerinde klinik olarak anlamlı bir iyileşme sunduğunu doğrulamıştır. Sonuç olarak, PRP tek başına veya kombinasyon tedavisi olarak EB fizyopatolojisini geri döndürmede gelecek vadeden, güvenli ve standart tedavi adayı bir modalitedir.

Erectile dysfunction (ED) is characterized by the persistent inability to achieve or maintain a penis erection sufficient for satisfactory sexual performance, and while conventional therapies like PDE5 inhibitors, vacuum devices, and penile implants exist, they offer temporary symptom management rather than correcting the underlying disease pathology. Consequently, intracavernous platelet-rich plasma (PRP) therapy has emerged as a promising restorative alternative due to its tissue-healing properties. Obtained by centrifuging the patient's autologous blood, PRP contains high concentrations of essential growth factors, such as VEGF, PDGF, FGF, and IGF-1, which stimulate angiogenesis, smooth muscle cell proliferation, and neural regeneration. Preclinical animal studies demonstrate that local PRP application effectively preserves myelinated axons and accelerates cavernous nerve recovery after injury, thereby improving erectile function. Furthermore, emerging clinical trials and retrospective analyses substantiate that PRP is a feasible and safe modality with minimal adverse effects, leading to significant increases in patient satisfaction and International Index of Erectile Function (IIEF-5) scores. Notably, predictive factors show that non-smokers and patients with higher baseline scores achieve substantially better therapeutic responses. Recent double-blind, randomized, placebo-controlled trials confirm that PRP provides a clinically significant improvement over saline controls, suggesting its immense potential to become a standard standalone or combination restorative treatment for ED.
