Hemşirelikte Klinik Liderlik
Özet
Sağlık kurumlarında düşük maliyetle yüksek kaliteli ve güvenli bakım sunma baskısı, etkili liderliğin önemini artırmaktadır. Hastane insan kaynaklarının %30-40’ını oluşturan hemşireler, sağlık hizmetlerinin kalitesini ve maliyet etkinliğini yükseltmede kilit role sahiptir. Literatürde yatak başındaki hemşirelerin liderlik potansiyelini resmi idari rollerden ayırmak amacıyla kullanılan klinik liderlik kavramı, başkalarını etkileyerek bakımı sürekli iyileştirmeyi ve doğrudan hasta bakımına katılımı ifade eder. Resmi liderlerin aksine, klinik liderler meşruiyetlerini idari yetkiden değil, sağlık ekibinin desteğinden alırlar. Klinik liderliği desteklemek için bilişsel, kişilerarası ve içsel (psikolojik güçlendirme, duygusal zeka, eleştirel yansıtma) olmak üzere üç temel yetkinlik çerçevesi ele alınmaktadır. Birleşik Krallık Ulusal Sağlık Sistemi (NHS) ise kişisel özellikler, başkalarıyla çalışma, yönetim hizmetleri, hizmetleri iyileştirme ve yön belirleme olmak üzere beş temel alan tanımlamıştır. Alanyazında Cook, Leigh, Pepin ve Supamanee gibi araştırmacılar tarafından çeşitli klinik liderlik modelleri geliştirilmiştir. Klinik liderlik hemşireler için nispeten yeni bir kavram olduğundan, bu becerilerin geliştirilmesi adına lisans ve lisansüstü müfredatlara eğitimlerin dahil edilmesi, klinik rehberlerin hazırlanması ve hemşirelerin psikolojik olarak güçlendirilmesi büyük önem taşımaktadır.
The pressure to provide high-quality and safe care at low costs in healthcare institutions increases the importance of effective leadership. Nurses, who constitute approximately 30-40% of hospital human resources, play a key role in enhancing the quality and cost-effectiveness of healthcare services. The concept of clinical leadership, used in literature to separate the bedside leadership potential of staff nurses from traditional administrative roles, refers to a process where nurses continuously improve care by influencing others and directly participating in patient care. Unlike formal leaders, clinical leaders gain legitimacy from the support of the healthcare team rather than assigned authority. To support clinical leadership, three main competence frameworks are addressed: cognitive, interpersonal, and intrinsic (psychological empowerment, emotional intelligence, critical reflection) competencies. Additionally, the UK National Health System (NHS) has defined five core domains: personal qualities, working with others, managing services, improving services, and setting direction. Various models have been developed by researchers like Cook, Leigh, Pepin, and Supamanee to foster this capability. Since clinical leadership is a relatively new term for nurses, it is highly crucial to integrate leadership training into undergraduate and postgraduate curricula, develop clinical guidelines, and structurally as well as psychologically empower nurses.
