Çocuğun Duygusal İhmali ve Hemşirelik Yaklaşımı

Özet

Çocukluk döneminde duygusal ihmal, ebeveyn veya bakıcının çocuğa gerekli sevgi, şefkat ve psikolojik desteği vermemesiyle karakterize, tanımlanması ve tespiti en zor kötü muamele türlerinden biridir. Küresel yaygınlığı %18.4 ile %73.9 arasında değişen bu durum, genellikle diğer istismar türleriyle karıştığı için ampirik veriler sınırlıdır. İhmalin ortaya çıkmasında istenmeyen çocuk olma, ebeveynin genç yaşta olması, ekonomik sorunlar, düşük sosyoekonomik düzey ve kültürel faktörler gibi çocuk, aile ve toplum kaynaklı birçok bağımsız veya etkileşimli risk faktörü rol oynamaktadır. Beyin gelişiminin en hızlı olduğu bebeklik döneminden itibaren başlayan duygusal ihmal; limbik ve kortikal fonksiyonların az gelişmesine, dilsel, bilişsel, davranışsal ve empatik sorunlara yol açabilmektedir. İlerleyen yaşlarda ise depresyon, kaygı, yeme bozuklukları, problemli telefon kullanımı ve oyun bağımlılığı gibi ciddi fiziksel ve psikolojik etkiler yaşam boyu süren bir kısır döngü yaratmaktadır. Bu döngünün kırılması amacıyla multidisipliner bir anlayışla hareket edilmeli; sağlık merkezleri, okullar ve ilgili dernekler iş birliği yapmalıdır. Özellikle ebeveyn-çocuk teması yüksek olan pediatri hemşireleri; ebeveyn eğitim programları, web tabanlı uzaktan eğitimler ve ev ziyaretleri aracılığıyla riskli grupların saptanmasında, toplum bilincinin artırılmasında, ihmalin önlenmesinde ve erken müdahale süreçlerinde anahtar bir rol üstlenmektedir.

Emotional neglect in childhood is one of the most difficult types of maltreatment to define and detect, characterized by a parent or caregiver failing to provide the necessary love, affection, and psychological support to the child. Although its global prevalence varies between 18.4% and 73.9%, empirical data remain limited as it is frequently intertwined with other forms of abuse in research. Various independent or interacting risk factors stemming from the child, family, and society—such as being an unwanted child, young parental age, economic difficulties, low socioeconomic status, and cultural beliefs—play a role in the emergence of neglect. Beginning from infancy, when brain development is most rapid, emotional neglect can lead to underdeveloped limbic and cortical functions, as well as linguistic, cognitive, behavioral, and empathic difficulties. In later ages, serious physical and psychological consequences such as depression, anxiety, eating disorders, problematic mobile phone use, and gaming addiction create a lifelong vicious cycle. To break this cycle, a multidisciplinary approach must be adopted, ensuring collaboration among healthcare centers, schools, and relevant organizations. Particularly, pediatric nurses, who have high contact with children and parents, play a pivotal role in identifying at-risk groups, raising public awareness, preventing neglect, and managing early intervention processes through parent education programs, web-based distance learning, and home visits.
