Çocuk İstismarının Psikiyatrik Yönleri ve Hemşirelik Yaklaşımı
Özet
Çocuk ihmal ve istismarı, bireylerin yaşam boyu ruh sağlığını olumsuz etkileyen küresel bir toplumsal sorundur. Son yirmi yılda yapılan araştırmalar, çocukluk çağı travmalarının majör depresyon, bipolar bozukluk, anksiyete, travma sonrası stres bozukluğu, dissosiyatif bozukluklar, bedensel belirti bozuklukları, yeme bozuklukları, kişilik bozuklukları, şizofreni ve alkol-madde kullanım bozuklukları gibi majör psikiyatrik hastalıklara yol açtığına dair güçlü kanıtlar sunmaktadır. Bu travmatik deneyimler, hipotalamik-hipofiz-adrenal eksenini aktive ederek prefrontal korteks, amigdala ve hipokampus gibi beyin yapılarında kalıcı olumsuz değişikliklere sebep olmakta, bellek, öğrenme ve duygu düzenleme mekanizmalarını bozmaktadır. İstismar mağduru çocuklarda intihar düşüncesi ve kendine zarar verme riski de önemli ölçüde artmaktadır. Bu bireylere bakım sunan hemşirelerin; güven ortamı sağlama, empatik yaklaşım, anksiyete ve uyku takibi ile travma odaklı müdahaleler gerçekleştirme gibi kritik sorumlulukları bulunur. İstismarı önlemek için istismarcı profilinin ve psikiyatrik yönlerinin (şizofreni, antisosyal kişilik bozukluğu, pedofili vb.) bilinmesi hayati önem taşır. İstismarcıların sıklıkla geçmişlerinde kendi maruz kaldıkları travmalar ve çarpık aile yapıları mevcuttur. Sonuç olarak, çocuk istismarının hem mağdur hem de istismarcı açısından psikiyatrik boyutlarının anlaşılması, nesiller arası aktarımın önlenmesi ve koruyucu ruh sağlığı çalışmalarına yön verilmesi açısından elzemdir.
Child neglect and abuse is a global social problem that adversely affects individuals' lifelong mental health. Research over the last twenty years provides strong evidence that childhood trauma leads to major psychiatric disorders such as major depression, bipolar disorder, anxiety, post-traumatic stress disorder, dissociative disorders, somatic symptom disorders, eating disorders, personality disorders, schizophrenia, and alcohol-substance use disorders. These traumatic experiences activate the hypothalamic-pituitary-adrenal axis, causing permanent negative changes in brain structures like the prefrontal cortex, amygdala, and hippocampus, which impairs memory, learning, and emotion regulation mechanisms. The risk of suicidal ideation and self-harm also increases significantly in abused children. Nurses providing care to these individuals have critical responsibilities such as establishing a safe environment, displaying an empathetic approach, monitoring anxiety and sleep, and implementing trauma-informed interventions. To prevent abuse, it is vital to understand the abuser profile and their psychiatric aspects, such as schizophrenia, antisocial personality disorder, and pedophilia, as abusers often have histories of their own trauma and distorted family structures. Consequently, comprehending the psychiatric dimensions of child abuse from the perspectives of both the victim and the abuser is essential for preventing intergenerational transmission and guiding preventive mental health efforts.
