İntrauterin gelişme geriliği (İUGR), fetüsün gebelikteki olumsuz çevre nedeniyle genetik büyüme potansiyeline ulaşamaması durumudur ve gebeliklerin %10’unu etkilemektedir. En yaygın nedenleri maternal vasküler malperfüzyon, fetoplasental dolaşım bozuklukları ve plasental yetmezliktir. Teşhiste, ultrasonografik tahmini fetal ağırlık (TFA) veya karın çevresi (AC) ölçümlerinin 10. persentilin altında olması kriter alınır. İUGR, 32. gebelik haftasından önce veya sonra ortaya çıkmasına göre erken ve geç başlangıçlı olarak ikiye ayrılır. Erken başlangıçlı İUGR, preeklampsi ve ciddi plasental hastalıkla ilişkili olup yüksek perinatal mortalite riski taşırken klinik yönetimi zordur. Geç başlangıçlı İUGR ise tüm vakaların %70’ini oluşturur; plasental lezyonlar daha hafiftir ve standart umbilikal arter (UA) Doppler bulguları normal seyrettiği için tanısı oldukça güçtür, ancak gebeliğin son döneminde ani fetal bozulma ve ölü doğumlara yol açabilir. Çevresel faktörler, sigara/alkol kullanımı, genetik anomaliler ve maternal kronik hastalıklar risk faktörleri arasındadır. Yönetim ve doğum zamanlaması kararlarında; fetal sürveyans için Biyofizik Profil, UA, MCA, Duktus Venozus Doppler ve amniyotik sıvı hacmi kritik rol oynar. Doğum kararı, prematürite komplikasyonları ile intrauterin ölüm riskleri dengelenerek; bulguların şiddetine bağlı olarak 30. ila 39. gebelik haftaları arasında planlanır.

Intrauterine growth restriction (IUGR) is a condition where a fetus cannot achieve its genetically determined growth potential due to an adverse intrauterine environment, affecting up to 10% of pregnancies. The most common causes are maternal vascular malperfusion, fetoplacental circulatory disorders, and placental insufficiency. Diagnosis is based on an ultrasonographic estimated fetal weight (EFW) or abdominal circumference (AC) below the 10th percentile for gestational age. IUGR is classified into early- and late-onset based on whether it occurs before or after 32 weeks of gestation. Early-onset IUGR is strongly associated with preeclampsia and severe placental disease, carrying a high risk of perinatal mortality, making its clinical management challenging. Late-onset IUGR accounts for 70% of all cases; its placental lesions are milder, and because standard umbilical artery (UA) Doppler findings usually remain normal, diagnosis is extremely difficult, though it can lead to sudden fetal deterioration and stillbirth in late pregnancy. Environmental factors, smoking/alcohol use, genetic anomalies, and maternal chronic diseases are major risk factors. Biophysical Profile, UA, MCA, Ductus Venosus Doppler, and amniotic fluid volume play critical roles in surveillance and delivery timing. Depending on the severity of these parameters, delivery is planned between 30 and 39 weeks of gestation by balancing the risks of prematurity against intrauterine demise.
