Doğum öncesi fetusun iyilik durumunun değerlendirilmesi "Antepartum Fetal Değerlendirme" olarak adlandırılır ve temel amacı riskli bebekleri belirleyerek fetal ölümleri engellemektir. Amerikan Obstetri ve Jinekoloji Cemiyeti (ACOG), perinatal sonuçlara etkisi net kanıtlanmasa da bu testleri öncelikle yüksek risk grubundaki gebelikler için önermektedir. Fetusun derin uyku hali gibi fizyolojik durumlar testlerin yorumlanmasında yalancı pozitiflik gibi sorunlara yol açabilmektedir. Antepartum değerlendirmede kullanılan başlıca yöntemler; annenin hissettiği hareketleri saymasına dayanan fetal hareket takibi, kardiyotokografi cihazıyla fetal kalp hızı ve uterin kontraksiyonları kaydeden Nonstres Test (NST), ultrasonografi ve NST parametrelerini birleştiren Biyofizik Profil (BFP) ile majör fetal damarlardaki kan akım hızlarını ölçen Fetal Doppler Ultrasonografidir. NST'de 20 dakikalık izlemde belirli akselerasyonların görülmesi "Reaktif" olarak tanımlanır ve nörolojik iyilik halini gösterir. Biyofizik profil, akut ve non-akut parametreleri değerlendirerek hipoksemi riskini belirler. Fetal Doppler ise özellikle umbilikal arter akımlarını inceleyerek yüksek riskli gebeliklerde perinatal ölümleri ve mortalite riskini belirgin şekilde azaltmaktadır. Sonuç olarak, düşük riskli popülasyonda yüksek duyarlılıkta tek bir test olmasa da, riskli gruplarda umbilikal arter Doppler ölçümlerinin takibe eklenmesi kritik önem taşımaktadır.

Antenatal evaluation of fetal well-being during the prenatal period is known as "Antepartum Fetal Assessment," with the ultimate goal of identifying fetuses at risk of damage or mortality in utero to prevent fetal deaths. Although there is no concrete evidence that these tests definitely improve perinatal outcomes, institutions like ACOG primarily recommend them for high-risk pregnancies. Physiological conditions, such as the fetus spending 25% of its time in deep sleep during the third trimester, pose challenges like false positivity in test evaluation. The primary methods used in antepartum assessment include fetal movement monitoring based on the mother counting perceived movements, the Nonstress Test (NST) which records fetal heart rate and uterine contractions via cardiotocography, the Biophysical Profile (BPP) combining ultrasound and NST criteria, and Fetal Doppler Ultrasound measuring blood flow velocities in major fetal vessels. In NST, the presence of specific accelerations within a 20-minute window is considered "Reactive," indicating neurological and cardiovascular health. The Biophysical Profile evaluates acute and non-acute parameters to assess the risk of central nervous system hypoxemia or acidemia. Fetal Doppler, particularly umbilical artery analysis, significantly reduces perinatal mortality by detecting decreased, absent, or reversed end-diastolic flow in high-risk pregnancies. Consequently, while no single test offers high sensitivity for low-risk populations, incorporating fetal umbilical artery Doppler measurements is highly effective for managing high-risk groups.
