Kolposkopi
Özet
Kolposkopi; vulva, vajina, serviks ve anüsün kamerayla büyütülerek incelenmesi ve şüpheli alanlardan biyopsi alınması işlemidir. İlk kez 1931'de tanımlanan bu yöntem, günümüzde preinvazif lezyonların tanınmasında ve doku tanısı konulmasında etkin rol oynar. Anormal servikal sitoloji veya HPV pozitifliği en sık endikasyonlarıyken; enfeksiyonlar, gebelik ve kanama diatezi rölatif kontrendikasyonlar arasındadır. İşlem öncesinde hastanın detaylı anamnezi alınarak aydınlatılmış onamı belgelenmelidir. Muayenede ışıklı bir binoküler mikroskop olan kolposkop kullanılır. Uygulama sırasında serviks temizlendikten sonra %3-5'lik asetik asit uygulanarak asetowhite epitel (AWE) varlığı değerlendirilir ve gerekirse Lugol (Schiller) solüsyonu ile yeşil filtre uygulamalarından faydalanılır. Bulgular normal ve anormal olarak sınıflandırılır; lökoplaki, mozaik görünüm, punktuasyon ve atipik damarlanma anormal işaretlerdir. Lezyonların ayrımında 0-8 arası puanlama sunan Reid Kolposkopi İndeksi kullanılır. Kolposkopik izlenim; normal, düşük grade, yüksek grade ve kanser olmak üzere dört temel kategoride raporlanarak dokümante edilir.
Colposcopy is the examination of the vulva, vagina, cervix, and anus by magnifying them with a camera and taking biopsies from suspicious areas. First defined in 1931, this method currently plays an active role in recognizing preinvasive lesions and establishing tissue diagnosis. While abnormal cervical cytology or HPV positivity are its most common indications, infections, pregnancy, and bleeding diathesis are among the relative contraindications. Prior to the procedure, a detailed anamnesis of the patient must be taken, and informed consent must be documented. A colposcope, which is an illuminated binocular microscope, is used during the examination. During the application, after the cervix is cleansed, 3-5% acetic acid is applied to evaluate the presence of acetowhite epithelium (AWE), and if necessary, Lugol's (Schiller) solution and green filter applications are utilized. Findings are classified as normal and abnormal; leukoplakia, mosaicism, punctation, and atypical vascularization are abnormal signs. The Reid Colposcopy Index, which offers a scoring between 0 and 8, is used to differentiate lesions. The colposcopic impression is documented and reported in four main categories: normal, low grade, high grade, and cancer.
