Özet

Temporomandibular eklem (TME), çiğneme, konuşma ve esneme gibi fonksiyonları kontrol eden kompleks bir yapı olup, bu bölgede görülen temporomandibular bozukluklar (TMB) kronik orofasiyal ağrıların yaygın nedenlerindendir. TMB’nin etiyolojisi; travma, parafonksiyonel alışkanlıklar ve psikososyal faktörler gibi multifaktöriyel risk unsurlarına dayanmaktadır. Hastalığın teşhisinde ayrıntılı anamnez ve klinik muayene (palpasyon, hareket açıklığı ölçümü vb.) büyük önem taşır. Protetik diş tedavisi, TMB yönetiminde oklüzal splintler ve protetik restorasyonlar aracılığıyla non-invaziv veya minimal invaziv tedavi modelleri sunar. Tedavide yaygın olarak kullanılan ortopedik apareylerden stabilizasyon splintleri kas ağrılarının giderilmesi ve parafonksiyonel aktivitenin azaltılmasında; anterior konumlandırma splintleri ise disk deplasmanları ve eklem seslerinin tedavisinde endikedir. Ayrıca ön/arka ısırma plakları, pivot ve yumuşak splintler de belirli endikasyonlara göre kısa dönemli tedavi alternatifi oluşturur. TMB tedavisinde agresif ve geri dönüşümsüz prosedürlerden kaçınılmalı, öncelikle hasta eğitimi, farmakoterapi ve fizik tedaviyi içeren multidisipliner, konservatif bir yaklaşım benimsenmelidir. Ağrı kontrol altına alındıktan sonra, hastanın fizyolojik ihtiyaçlarına uygun oklüzal tedavi ve protetik restorasyonlar ile çiğneme sisteminde fonksiyonel denge yeniden kurulmalıdır.

The temporomandibular joint (TMJ) is a complex structure controlling functions like mastication, speech, and yawning, and temporomandibular disorders (TMD) affecting this region are common causes of chronic orofacial pain. The etiology of TMD is based on multifactorial risk elements such as trauma, parafunctional habits, and psychosocial factors. Detailed anamnesis and clinical examination (palpation, range of motion measurement, etc.) are of great importance in diagnosis. Prosthodontics offers non-invasive or minimally invasive treatment models in TMD management through occlusal splints and prosthetic restorations. Among the widely used orthopedic appliances, stabilization splints are indicated for relieving muscle pain and reducing parafunctional activity, while anterior repositioning splints are used for treating disc displacements and joint sounds. Additionally, anterior/posterior bite planes, pivoting, and soft splints provide short-term therapeutic alternatives depending on specific indications. Aggressive and irreversible procedures should be avoided in TMD treatment, and a multidisciplinary, conservative approach including patient education, pharmacotherapy, and physical therapy must be adopted primarily. After the pain is controlled, functional balance in the masticatory system should be re-established with occlusal therapy and prosthetic restorations tailored to the patient's physiological needs.
